Guide for TT-Nirvanna-Rank IX Progression?

I'm looking for a guide that shows how to progress from the TT gear to the end game weapons and armour. I'm pretty comfortable with the TT gear (thank you Stolen Knowledge Wiki) but the order and requirements for getting higher gear boggles me. Please point my in the right direction.
