BH59 help..?

How do I "stack" BH59?

Why is it that sometimes the squad has to be dismissed and reinvited to "reset the dungeon"?

What about chains? People seem to know what bosses they for BH2, BH3? Is the sequence of the bosses specific in different chains, and each sequence is chosen everyday for us to do?

I just hit lvl70 not long ago, and I'm really confused now... b:surrender

Hope someone can clarify these 3 questions for me.

    somebody needs to kill something that already got killed. resetting the dungeon lets you start it over with all the mobs and bosses alive again.
