about berserk

I was reading about this attribute but i've some doubts. Is it an attribute of some weapons or you can put berserk in weapons with something or...? how works that? i know you lose 5% HP but for exemple in axes i only see it in calamity before lvl70 but i saw pics of people below 70 talking about berserk with axes.

    1) You can't put it in a weapon, it is a random add on on some weapons (normal crafted/mob dropped ones, Nirvana and OHT) and fixed on some TT/Legendary equips like TT90 gold axe/sword and Calamities. That list is incomplete though.
    2) You lose 5% HP and get double damage on that hit.
    3) I've seen level 40 spear with berserk, as well as 37 axes.

    It's an addon that can come on just about any crafted melee weapon, though it's pretty rare.

    The calamity axes you mentioned always have it though, which is why they're so popular.
