You can craft weapons at any blacksmith, as long as you have the skill to do so. You get this skill really early on (lvl5) from a simple quest. From then on, just go to any blacksmith, click on manufacture weapons, and see what materials you need to make the one you want. Each weapon needs different materials, and in different amounts. How you get those mats is up to you though.

I guess by gems you mean souldgems? These are more commonly referred to as 'shards' in game. These shards are items that give stat bonuses to weapons and armor, depending on type and level. For example, garnet shards will add physical defense to armor and attack to weapons, and the higher the grade, the better the bonus. The most commonly used types are probably garnet (physical def/atk), sapphire (magiac atk/def), and citrine (hp). Although Topaz (evasion/accuracy) is also in fairly common use. The shards that give bonuses to a specific element (i.e ruby and aquamarine) are really only useful for a daily quest called Crazy Stone, which starts at lvl 30.

You can put shards in weapons and armor as long as they have a socket for them. You can tell if they do or not because it'll say it has X number of sockets right next to the name. Take a shard and the equipment to the appropriate NPC (Blacksmith for weapons, Tailor for armor), select Imbue Soulgem, drag the equipment and shard to the slots they go in, and hit imbue. There is a cost involved in doing this, so be aware of that. Also, be aware that armor can have a maximum of 4 sockets, and weapons can have no more than 2.
