Problems with call to duty, 39/ gates of delirum

So I am currently doing Call fo Duty 39 Gates of Delirum
so thats 3 CtDs
kill
- Cereneti
-Calcid
-Trioc

Being the leader, I was exchangeing the CtDs for the quest at the in instance pillars (entrence of Dungeon)

Cereneti and Calcid worked perfectly, however my attemps at Trioc kept on receiving "failed" and the CtD was returned to me
what could cause this?

    The only thing I can think of is that someone's quest list might be full. I'm pretty sure that would give you the Failed status. Someone might have to trash a quest, then try again.

    Get another squad and go do Trioc and don't forget to die later on.
    Looking for a decent casual understanding Faction.

    Are the people helping doing BH? If they're leaving instance/squad to get next part that'll fail the Call to Duty.

    strange thing is

    all my squad mates got it
    and my i had only 15/20 quests
    and when i killed the thing it was marked as complete
    so it looks like the "failed" was a visual glitch

    allthou I did find a second glitch
    when I killed calcid (not he boss with the initial glitch) the stone showed the quest sign again, so I was able to reopen call to duty later on it timed out

    so as i stands, I have all 3 call to duty completed and exp awarded, othewr quests done
    and i have a call to duty for one of the 3 bosses giving me an opertunity towards more xp
