Do away with the Sword/Shield Switch in PVP

At least for the defender who is likely already at a severe disadvantage being a victim of the element of surprise, to suddenly having to fumble around switching off checkmarks or trying to remember to hold ctrl key down is silly. If you are attacked I think you should be switched into PVP automatically, unless by a guard.

I know you can avoid all this by just staying in PVP mode, but that simply invites attack, and defeats the whole point.
