Stats Conversion Question

I figured out that, at least against monsters, a blademaster should be able to become unhittable with high enough dodge rating. From what I've read, dodge rating comes purely from agility.

However, I had two questions concerning this:

1) Does Evasion add to your dodge rating or your %, or does it factor into a separate calculation (that is, from (100 - Monster's Base Aim %) * ((1 / 2) ^ ((Dodge Rating / 25) - 1)) = Dodge %).

2) Do Dodge and Aim %s use these calculations in determination of Dodge and Aim %s against other players, and if so, what are the "Monster's Base Aim/Dodge %s?" :

(100 - Monster's Base Aim %) * ((1 / 2) ^ ((Dodge Rating / 25) - 1)) = Dodge %

(100 - Monster's Base Dodge %) * ((1 / 2 ) ^ ((Attack Rating / 25) - 1)) = Aim %
