rebirth 2 requirements

post is pretty self explanatory, just wondering what are the requirements for rebirth 2 other than lvl 100, so culti level, astral level, quest chains, etc. It's been a while since I've had to think about rebirth, so kind of forgot about it until the new email code. Thanks for any help!

Normally you need Arcane Sky VI for the 2nd rebirth, but if you used the code for pack, it comes out with the 2nd rebirth stone, so you can do it right away as you hit Lv100 on rb1.
