Why does some loot and rewards from chests and such have a +1, +2, +whatever on it when you look at what it is in the chest or reward screen but when you actually get the item the "+" disappears? I am an old school D&D player and when i see a + anything i get all excited. Then it turns out to be nothing. Is this a bug or am i just hallucinating again?

The +# indicates the item's tier. I think +6 or +7 is equivalent to tier 2.

This is correct. I think 6 is T2 and 7 is T2.5, However like OP mentions, I too am curious as to why it disappears after acquiring it. Hard to keep track of which pieces are from which Tier.
