I had a code which gave me a pack that was supposed to have an epic quality weapon for my class and level, so I used it at 60 and there was no weapon in it, just the xp, greater stone of health and Astral Diamonds.
It would help to know where you got the code and what the name of the pack was. The items you received also come with other packs, but they are "once-only". The primary items that come with these packs are often delivered in mail, but also at the reclaims agent in Protector's Enclave.
At the Reclaims Agent there are several tabs, one is to retrieve full boxes of your stuff, and another to take individual items.
In short, if you were supposed to receive a weapon, that is likely where you go to claim it. (It sounds like the Delzoun weapon that also comes with the Guardian and Hero of the North packs - which is claimed at the Agent).

To this end, be aware that unless your interface UI scale is set to 1.0, you can't see all the items at the claims agent. This is a new bug with vendors since mod 4. If you suspect that this is happening to you, go into interface options, and temporarily drop UI scale to 1.0. Talk to the vendor (in this case, the claims agent), and then after you conclude your business reset your interface scale to whatever it was before.
(Yes, this may not be the problem, but worth a shot)
