Here is a list of bugs I have seen since Tyranny was released:
Enchants don't stack without you having to use those small bag icons on the bottom of the inventory screen. I don't think placing an enchant on top of another enchant means we want to start the refining process. If this is intentional please undo this effect it is very aggravating.
In the character sheet screen on companions the bottom most enchantments do not show the progress bar for how much you have refined the enchantment.
Barkshield sometimes does not really show up at all just leaves this very vague white outline. This might not be bad as I don't really care for the barkshield look (I like the effect but the look isn't all that nice).
When I bring up any window in the game that covers the daily power die indicator, the daily power indicator constantly flickers throw the window.
I'm sure there is more but that is all I can remember at the moment. If anyone has any others please list them, don't know if it will help resolve them but these are a bit annoying but not exactly game breaking.

Please mention the biggest bug of all. The one crashing the game at the load screen. Preventing many people from even getting in the game.
On this last one, I had it happen once, but not again even after doing numerous dungeon runs. Though the group (guild runs) rotated between different dungeons or we were running CN, which is lengthier than most (maybe all) T2's.
There are also visual glitches on screen. Hard to notice, but annoying if you notice them once then you see them all the time.
Also Artifact icon sometime blinks like crazy.
The new "feature" - showing how much XP is needed for next level in character screen, shows same numbers all the time.
Gateway hangs more often that before, but this can be counted as old bug, as this was happening long before mod4.
Regular crashes when leaving dungeon (i notice this in FC, as I play it many times every day, but i think it happens to other dungeons too) This was happening before mod4 but not as often as now.
In VT tower wrights, or whatever those things which appear along with red zones are called, don't follow you if you don't get too close to them. maybe it is not a bug but new feature - please don't fix it
In new dragon HEs, dragons sometimes go above ground - looks like flying but without moving wings - it is obvious wrong positioning - ugly but not showstopping.
Some of encounters (probably dailies too) if interrupted in specific time, do not go on cooldown and can be cast again immediately. They do damage both times. It is hard to reproduce it intentionally, but i noticed it more that once, so it is probably happening. If someone learns how to do it, that would mean unlimited encounters all the time
Some epic boss drops for SW gear is BOP - this could be intentional.
For obvious reasons, I don't wont to mention obvious errors in damage calculation for some classes ...

GFs able to stack Knight's Valor if they have different amount of points in the skill and using features/feats/enhancements to completely wipe PvP/PvE content.
1. Unfinished dragging of enchantment to refining window causes to replace Item to refine with the half-dragged enchantment.
2. On the Eye of Lathandar right-click menu, Discard is listed above Refine option (which got pushed to the bottom), unlike in most other refine items, which can cause confusion and unintentional loss of the artifact. (Do we really need Discard in menu options btw? Maybe leaving it through only drag&drop would be enough and less risky).
3. Dread Spire - Servitors spawning when fighting the boss (starting from the 2nd wave) no longer run to attack the player but attack from a distance now, which is quite pesky for melee classes.
4. Angel of Protection companion at dragons runs right up to the dragon and stays there till killed (aka till first dragon AOE attack), without using healing skills, even if the owner is ranged class staying far away from the dragon. Sometimes it follows random players around too and not the owner.
5. The lowest runes in the companion window do not show the refining progress - their display is cut right at the edge under which the progress bar should be.
6. There seems to be an issue with GF block / tenacity vs CC effects - some effects seem to pass through the block and there is no noticeable difference in pvp in cc skills duration with and without profound set on, although pvp is such a mess atm that it's hard to tell anything for sure.

Merothrax also drops the helm fashion instead of the Chest listed on the Ctl-j pane, As someone who has farmed this dragon only to hear the chest doesn't drop from him like it should.
I personally have received the helm fashion from him, when his friend in Ebon downs also drops the helm, Though that is listed in the ctl-j pane as accurate.
While the issue may not be as serious as some of the others listed here, i do want you to know that Merothrax the greedy is not dropping the right item and should be fixed whenever possible.
I have a bug where the button to claim a reward from a Heroic Encounter isn't working. Hitting ctrl+f also does nothing. It happened in Roethe Valley after defeating the dragon. I had completed it dozens of times before it glitched.
Rogue walk animation is just a slow-motion run now. Friend brought that one up.

I am still unable to get a 2nd Greater Bag of Holding (24 slots) and an Adventurers Helper Pack even though I bought both the Hunter Ranger Booster Pack AND the Scourge Warlock Booster Pack. It said that this was now fixed? Is it not? I'm doing this on a brand new Dragonborn Scourge Warlock character, so it should give them to me, I would think?

The shader issues have a likley fix for at least a god chunk of cases this week and preview today.
