That said, none of them are really difficult once you get some experience in each of them and run with a competent party. (note: COMPETENT, not overgeared, elite, uber, or whatever, just competent.)
I like how nobody has actually listed in the order requested.
Let's see, hardest to easiest, all the epic version:
Castle Never (Haven't been in a party able to clear Azharzel and the hands always get me.)
Dread Vault (Annoying as hell the whole way through and a boss fight that takes aeons.)
Valindra's Tower (Short and sweet, but see Azharzel, and I have the WORST luck with the wraiths in the 'run in circles' phase. )
Malabog's Castle (Most parties don't have the DPS to burn Valindra fast enough when she comes down)
Caverns of Karrundax (Actually one of the easiest final boss fights ever, and an easy second boss, but Pyraphenia takes down so many otherwise competent groups unless the group has the DPS to burn her faster than the Forgecaller can kill the party when he shows)
Temple of the Spider (Easy dungeon, but the final boss is easy to difficult depending on composition, skill, and DPS.)
Spellplague Caverns (Easy dungeon, easy final boss. People mistake 'long and tedious' for 'hard'. A halfway competent party and this is stupidly easy.
Frozen Heart (In a competent party. PUGs will wipe endlessly at Grast because they can't figure out the orbs. Hrimmy can be a pain, but all in all, not too bad.)
Grey Wolf Den (Tedious. Ethraniev is a female dog. Literally.)
Mad Dragon (Most PUGs wipe on the first subboss, the ones that don't wipe at the Pit Fiend. Final boss isn't bad, just getting there is irksome)
Lair of the Pirate King (This shouldn't even be a Tier 2. Seriously easy boss, the only challenge is as a TR trying to FIND the Redolent Surgeon in the mob of adds that are all taller than he is. :P)
Throne of Idris (Easy.)
Cragmire Crypts (Even easier.)
The Cloak Tower (If you can't beat this dungeon you should probably go cry in the corner out of shame.)
Largely inactive, playing Skyforge as Nai Calus.

I'm sure people have before (or maybe not) but basically everyone who's done EDV has done it the cheese way which involves only ranged on boss, fighting in the back corner and hiding to avoid thoons as they despawn. long slow kill still involving tons of adds and 2 minibosses which are basically any t1/t2 boss from any previous dungeon. People have been clearing cn the "right" way (ie not throwing adds) for a long, long time, while, and again i'm sure it has happened, i've yet to see or hear of anyone clearing EDV the right way, and until that happens with the regularity that CN clears happen EDV will remain the hardest dungeon bar none.
That's why I listed FH twice. it's much more difficult and yet fun if you deal with the adds instead of having someone simply kite them all and either wipe afterwards or clear them.

never done DV =P, no reason to do it at all... and people say it's boring.

Cannot really comment on Castle Never (Only completed this one once recently due to hefty bugs, like too many people trying to go through a door locking everyone out, bosses not spawning, etc, I don't even try to do this one anymore unless someone I know really really wants to. But if I rate it according to how many times bugs stopped my party, it would be at the top. But it is a mass swarm of mobs that requires a lot of AoE support to get through, I do see why people think it requires a 3CW party.)
1. Dread Vault T2 (This one I've seen absolutely smash a DPS designed party and any and all Melees. When I go through this one, I have to run a total control wizard setup, and the ending boss requires a good 2-3 CWs going all out control due to the number of adds, while the rest use distance attacks on the brain. Not doable by melees due to the Thune Hulks crushing them. And meleeing too close to the brain Keeps the Thunes there rather than by the edges where they have to be gathered up and tossed off the back side. Best party is usually Control Wizards and Rangers)
2. Malabog's Castle (Yes, getting a party with DPS and then alert enough to take out Valindra fast enough when she pops in the Dragon fight is nearly a 1 in 10 chance. Which hoses up the entire run no matter how good you've done up to that point, Easy to get there though with a lot of AoE as you get mob swarmed a lot but they are weaker, don't need a 3CW team for this one.
But the Boss Fight requires major single target Distance DPS.... rare to find in a party...... still lately this one has been extremely buggy, Valindra Choking people from inside walls, Summoning Portals on her Ledge no one can get to... etc.)
3. Valindra's Tower (I think the actual run up there is tougher than the actual boss fight, mass mob attack with stronger mobs this is a Multiple CW/GWF party to be sure. But the end battle is the complete opposite, it's identical to the Malabog Dragon Fight - minus the Dragon, which is surprisingly easy, but the party needed to complete has to be speedy, not slow, and one non alert person hoses everything, which is ironic as it requires a lot of AoE DPS to get there, and none to complete)
4. Temple of the Spider (This boss, can be almost impossible without just the perfect party composition and serious skill. Getting there, however is relatively easy, no mass mob attacks, lots of ledges to bump or throw things off. No real major Mass Mob storms makes getting there easy for just about any party... the boss battle though is a major add fest, and a tough add fest at that.)
5. Caverns of Karrundax (Actually one of the easiest final boss fights ever, and an easy second boss, but Pyraphenia takes down so many otherwise competent groups unless the group has the DPS to burn her faster than the Forgecaller can kill the party when he shows. Pyraphenia is the toughest boss fight in the dungeon.)
6. Spellplague Caverns (Easy dungeon, easy final boss. People mistake 'long and tedious' for 'hard'. DOES require a lot of AoE support though. A halfway competent party and this is stupidly easy. The only Boss even worth worrying about is Mouth of Madness, but that's because its a major add fest that can't be CC'd)
7. Frozen Heart (Major AoE required dungeon, needs lots of that to get there. Just a very long end boss fight which requires the complete opposite tactics you've used up to that point.)
The rest are just too easy for my character at this point, so the only time I run them is for fun, just to relax, and perhaps even help some newbies with tactics and getting through them.
2. Malabog's Castle (Yes, getting a party with DPS and then alert enough to take out Valindra fast enough when she pops in the Dragon fight is nearly a 1 in 10 chance. Which hoses up the entire run no matter how good you've done up to that point, Easy to get there though with a lot of AoE as you get mob swarmed a lot but they are weaker, don't need a 3CW team for this one.
just make sure everyone counts voids to three...
major dissapointment. easiest bossfight ever before the maelstrom nerf, and even now not that much of a challange (granted you have a t2 equipped party).
VT with a team of guys who understand that opening coffers wins the battle is allso suprisingly easy.
the only hard encounters in any t2 dungeon i can think of are the pyrolady from karru, the spiderqueen and the FH endboss if you lack a kiter.
except for mallaborg, vt and sp the encounters are all just addfests anyways.
well ok the zombie madness in sp can be rrrreally clunked up too, but its at least one of the few original encounters.
there is no luck involved in avoiding wraiths in VT.

I agree with your order, but...
I don't know how you can claim kiting adds in FH isn't intended, has a dev come out and said this? Now, everyone dying at the end so all the adds despawn ... sure, that's not intended.
I've cleared DV legit before, though given your belief about kiting, you'd probably consider it not legit. The TR permas, and everyone else is ranged, didn't hide from thoons. GS 16k-20k matters
It's still harder than CN. And even if it wasn't, more people would clear CN regularly because CN drops are worth more overall.
I have ran Dread Vault twice in all my life, seeing no reason to go there, so I'll leave it off. I also have not yet unlocked VT.
1) Castle Never (This dump is holding my Cleric back as I need those weapons. Boss fight goes well till about 50% health then the adds get out of control and there's lots of yelling and then wipe.)
.
.
.
.
.
.
.
.
.
.
.
.
(Okay, a lot of that was out of frustration)
2) Malabog's Castle
.
.
3) Lair of the Mad Dragon (True story. I've never completed this at intended level. Only complete it if there are 2 or more geared T2 players.)
4) ToS (Only because of the last boss. Second boss dies faster than a Rimefire Golem/Maw)
5) Karrundax (Only because of Pyraphenia fight and 30% add spawn at the Hand fight)
6) Spellplague (More irritating than hard)
.
.
7) Grey Wolf Den
.
8) Pirate King (Should be called Add King. Bring 2 CWs = You win)
9) Frozen Heart (We leap most of our fresh 60 guildies straight into this one. A decent GF will carry an entire group of lowbies so long as they listen when told not to AOE)
.
.
10) Throne of Idris (Only harder than CC because a novice party that doesn't listen will aggro entire boss room and wipe)
.
11) Cragmire Crypts
.
.
.
12) Cloak Tower (If you can't do this uninstall the game and play Solitaire Online)
PSA: You don't need to grind Spinward Rise for your Elemental artifact main hand if you have some AD lying around. You can craft it via the Tyranny of Dragon's campaign screen.
It's kinda hard to say which is hard and why for me since some time ago I did 3 dungeons with really experienced players from legit community channel and now I belive everything is possible when you're properly geared and know what to do. Dungeons were: Temple of Spider, Frozen Heart and Spellplague; no tactics just pure, good old "kill'em all" playstyle and guess what? No wipes and for the first time in this game I saw FH final boss purge of every living thing.
But if I would have to point out then it would be:
1. Dread Vault
2. Castle Never (<^- thoose two are seriously sign of old days when devs had only one idea for bosses: "moar adds")
Rest (if you're undergeared or with people who have no idea what to do or/and don't follow/understand your instructions):
3. Valindra
4. Malabog
5. Spellplague
6. Frozen Heart
7. Karru
8. Pirate
Rest doesn't matter for me since I did them when having (old) 11k GS with similar or better geared players and really can't decide if i.e. Mad Dragon is harder than Idris

Category 1 - I have never been able to complete this
Dread Vault
Category 2 - requires people to know what they are doing - pretty much impossible with a random PUG
Castle Never
Valindra's Tower
Category 3 - hard, but doable with a PUG - easy with a coordinated and communicating guild group
Malabog's Castle
Category 4 - doable with most PUGs, unless people do really stupid things.
Temple of the Spider
Caverns of Karrundax
The Spellplague Caverns
The Frozen Heart - trivially easy with a lot of kiting at the end.
Category 5 - even easier
Gray Wolf Den
The Lair of the Mad Dragon
The Throne of Idris
1.) Valindra's Tower
Never ever even tried to enter.
2.) Malabog's Castle
Tried only once with a LegitCommunity team. We could not even pass the outside mobs.
3.) Castle Never
Went once with another Legit_Community team. Was smooth until the last fight where we consistently wiped, without removing even 5% of the boss' HP.
4.) The Temple of the Spider
Never beaten this one. Tried once, we were all new there except one guy who said he knew how to do it. Then he started to die again and again and he left and we wiped.
5.) Frozen Heart
Went there a couple of times. Never won, but came close a few times.
6.) Spellplague Caverns
Went once with my guild, we won without a single wipe but it was a lagfest from start to end.
7.) Gray Wolf Den
I thought for a long time this one was unbeatable.
8.) Mad Dragon
This one is a pain for a Melee. Glue everywhere on the ground, ranged adds.
9.) The Dread Vault
Boring as boring can be. Last fight is just rolling dices and getting OS-ed or not by a tentacle when running.
10.) Caverns of Karrundax
The last where you can die.
11.) Throne of Idris
12.) Pirate King
13.) Cragmire Crypts
14.) Cloak Tower
1)Dread Vault
Absurdly annoying to do for a terrible reward.
2)Castle Never
Need to know some shortcuts to save time, dungeon is pretty straightforward and not very difficult. Just the Draco fight makes this dungeon to be in spot number 2, which if you dont have the correct composition of experienced players you wont ever beat it.
3)Malabog Castle
I find this one more difficult than VT for just two reasons, Fomorians are a pain in the *** with that CC inmunity and if you arent careful they can instakill you and I think it takes a bit longer to complete than Valindra IMO. Draco fight can be tricky if you dont have single target burst to Valindra.
4)Valindra Tower
Most linear dungeon ever, it's literally just straight beat'em up. Just pack AoE and enjoy numbers pop. Boss is pretty easy if all members of your party know english and can communicate, can be done pretty safe if people are decent at popping caskets.
5)Spider
You need to have decent gear to take down final boss. If you dont have it you will suffer the wrath of blademasters and you wont be able to outdps her regen.
6) Karru, SP,FH
Tedious and boring dungeons. FH being less boring and more easy.
7) GWD, Mad dragon
GWD boss fight is more difficult than mad dragon. Mad dragon is pretty easy even with terrible gear if you strategize because it doesnt rely that much in adds as others.
8)Pirate king
Why is this even T2?
9)The rest
I dont even remember them.

HARDEST TO EASIEST (assuming all epics):
1. Dread Vault--EXP necessary. team make-up and ranged and dps/control skills.
2. Malabog's Castle--EXP necessary. team make-up and specific interrupt skills necessary for final fight.
3. Castle Never--EXP necessary. team make-up and skills necessary for mobs and last boss.
4. Valindra's Tower--EXP necessary. team skills necessary on last boss.
5. Temple of the Spider--EXP necessary. team skills necessary on last boss.
6. Spellplague Cavern--EXP necessary. need skills on 2 of 3 bosses (not including secret boss).
7. Frozen Heart--EXP necessary. need kiter. need skills on last boss.
8. Grey Wolf Den--EXP necessary, need skills for last boss
9. Karrundax--EXP necessary, need skills for 1st sub boss.
10. Mad Dragon--EXP necessary, need strategy on last boss/last room
11. Throne of Idris--EXP helpful, need strategy in last room
12. Pirate King--EXP helpful.
13. Cragmire Crypts--no EXP or special skills needed.
14. Cloak Tower--no EXP or special skills needed.
I'm not saying kiting isn't a valid tactic or that it isn't legit or anything. I'm saying when they designed the encounter, is it what they had in mind? "Ok so this fight, we'll have the GF run in circles for 20 minutes while the rest of the party dps the boss?"
Its hard to know what they had in mind, but when this dungeon was hard (back when we were all 8k gs) this was the "way" that we all figured out how to reliably beat it, 20min of the slowest class running in circles while the rest single target the boss.
But that surely couldn't have been what they intended? if so, then all the adds should despawn the moment the boss dies. otherwise, you run the risk of the loot despawning while you are clearing all the adds. Again was much more likely to happen when we were 8k gs. In fact wiping afterwards (and 1 player respawning and wiping a 2nd time) to clear all the adds kind of happened by accident, from the party trying to kill all the adds afterwards and failing to do so.
Now that players are far more powerful we can just aoe everything and don't need someone to kite, but it took longer to do this than it did to clear CN "Legit". Only the boss (hrimnir) is weaker than Draco, but the adds in FH (#, cc immunity, hp, and aoe which are worse than RW) are much worse than the adds in CN, which is the only reason that CN is harder than FH "as intended". hopefully that makes sense.
for EDV. Why bother coding the tentacles or thoons, having the thoons constantly respawn and tentacles respawn on phase change, if the intended way to defeat it was to run to back corner and hide from all of those things? Those are the tactics we use to defeat EDV, but has it ever been done "as it was imagined" ie fighting on the boss, and dealing with the thoons? killing or avoiding the tentacle?
And if it has been done that way before successfully, then it's certainly much harder than any other boss fight in the game. That was my point really.

The rest are just too easy for my character at this point, so the only time I run them is for fun, just to relax, and perhaps even help some newbies with tactics and getting through them.
major dissapointment. easiest bossfight ever before the maelstrom nerf, and even now not that much of a challange (granted you have a t2 equipped party).
1) Castle Never (This dump is holding my Cleric back as I need those weapons. Boss fight goes well till about 50% health then the adds get out of control and there's lots of yelling and then wipe.)

Dread Vault
Category 2 - requires people to know what they are doing - pretty much impossible with a random PUG
Castle Never
Valindra's Tower
Category 3 - hard, but doable with a PUG - easy with a coordinated and communicating guild group
Malabog's Castle
Category 4 - doable with most PUGs, unless people do really stupid things.
Temple of the Spider
Caverns of Karrundax
The Spellplague Caverns
The Frozen Heart - trivially easy with a lot of kiting at the end.
Category 5 - even easier
Gray Wolf Den
The Lair of the Mad Dragon
The Throne of Idris
Category 6 - far too easy, considering it's a T2
Lair of the Pirate King
Category 7 - the really, really easy ones
Cragmire Crypts
Cloak Tower
Epic Dread vault.
2) Haven't managed it, surely I can do it with the right setup:
Valindra Tower
Castle Never
3) A good fight with a good team
Malabog's Castle.
4) Can PUG it with a bit of luck:
Karrundax
Temple of the Spider
Mad Dragon
5) Easily PUGable
Frozen Heart
Spellplague (But this one is SO annoying!!)
Throne of Idris
6) Faceroll territory
Cragmire Crypts
Pirate King
7) HAHAHAHAHAHA
Cloak Tower
Guild--And the Imaginary Friends
