Not a party member while starting epic dungeon

It happend to me several times after update. While starting an epic dungeon I've arrived at site as a single person and not as a team member despite que system. System doesn't add me to the party automatically, everything has to be done manually.
This way I didn't complete no dungeon since update :(

    Same thing to me twice so far. The first time I could not leave the instance except to walk out and then I had the 'return to instance' and could not remove myself from it. I logged out and finally I was free.

    The second time was the same except that I have logged out and in repeatedly and am still stuck with the instance and unable to leave it.
