no avatar on death.

The no avatar on death issue is still happening. Happened again after patch. I died in purple environmental lava in spellplague caverns and when I spawned at the fire I had no avatar. I've heard others mention it is related to changing your armor skins. For reference, most of my equipment is reskinned. This is not new in this patch though. It has happened many times before today's patch.

    Also happened post patch in Frozen Heart and Temple of the Spider
