Loot roll never finished even though everyone made a choice

So I cleared CN with a pug group and the MH dagger dropped. Everyone rolled greed except me, I rolled need because it was an upgrade. The loot roll never finished and the dagger was never looted to me.
One odd thing is that someone mentioned that they were able to just walk up and loot a vorpal shard without a loot roll initiating. This made me wonder because one member of our party instantly left the group after rolling greed on the dagger and logged off.
What do you guys think happened? Did the loot just get bugged and lost? Maybe because someone left group during the roll....Or do you think the person who left group may have did a loot exploit?

I'd like to add another thing I just noticed. I never received the achievement for completing Castle Never nor did the cutscene ever play? Might that be part of the issues?
I sent in a GM ticket. Was a GM ever able to restore the item to you? I usually wouldn't care too much...but it is the best weapon in the game for my class and it took going through 4 fail groups before I finally got in one that could clear the dungeon.
