One orb is behind a pillar, the second orb is behind a tiny nook and seemingly behind an invisible outcropping as no one can get to it. both orbs are needed to get to Oculus before the final boss fight. Bugged or not implemented yet?
There is no worse feeling in the world than the moment during an argument you realize you are wrong.
I don't know...an hour ago I finished soloing my way to the doors, and after I put the first orb in it kept telling me, "Something is missing" when I tried to open the doors. I'll have to try it with two orbs, it seems.

We tried getting that tonight. You can get both orbs in the Heroic version, though the one in the Mouth chamber isn't in the same spot as in the Epic. However, we also encountered a bug which prevented us from getting through the door. Each of my two teammates grabbed an orb. Each attempted to place the orb in the same slot at the same time. Both orbs were used up but only one slot activated.

I finally managed it. The second orb was in the Mouth of Madness room, in a little cubby in the wall to the right of where you enter. Once I had both orbs I had no problems opening the door, though I had a problem entering the eye chamber. The adds inside aggroed such that I couldn't activate the door. I solved that by going all the way back to the campfire and returning, then I was able to get in.
The Oculus fight was very easy, but I had a problem afterward -- I couldn't leave the dungeon! Logging out and back in again simply returned me to the same instance. I decided to give the final boss a go, and the fight was...bizarre. Maybe it's because I had only done the epic run before, but it was as if all of the zombies just wanted to try intimidating me to death (or maybe they thought bad breath might kill me, who knows). I know that if you're on the boss they won't attack you, but none of them would attack me *or* my companion as long as the boss was up, no matter *where* we stood, and there were no frogmen (thank goodness!). Of course, since they weren't attacking me, my Split the Sky refused to send any lightning at them. Every once in a while I'd turn around to check on my cleric and she would have a few zombies huddled around her giving her every intimidating stance they could, but otherwise not lifting a finger toward her. Just to be safe I'd shoot them so they'd huddle around me, which left my cleric free to run up and attack the boss unmolested.
Definitely bizarre.
The fight turned out to be just me using Split Shot because of ~20 zombies huddled around me all getting in the way, with the occasional fight with them whenever the boss disappeared every 20%. I took lots of screenshots just because I was so amazed...

It is slightly out of date since you don't have to grab the first orb twice anymore (on normal, which is what you need for the achievement anyway).
