List of end-game armor and drop locations?

I was wondering if anyone's made a list of all the end-game gear, and which dungeons they drop from? The Wiki has a couple bits here and there, but is woefully unfinished.

    It all drops on the auction house. :p

    Yeah but when it drops in the Auction House it takes away Astral Diamonds from me :P
