How to get last two companion skins?

Does anybody know how to get these yet? The names of them seem to make them sound a lot different than the ones you get from getting the companion to max level.

    I presume you are talking about the white quality companions which you can buy with in game gold. Currently, those can only reach rank 15 but if you have a look at the merchants inventory (grand emporium, Protectors Enclave) you will see that there will be upgrade books for those companions (currently "coming soon") which will allow them to go past rank 15 - the greyed out skins are for those higher ranks.
