Dungeon Queue not working

Occasionally when I queue up for a dungeon, After I accept, I will not be teleported to the dungeon and I will be unable to leave the party and there is no return to instance button. I have to log out of the game in order to get out of the group. Some other people in my guild were having the same problem. I am on dragon server if that helps.

    More Info. If I zone out then I do get a return to isntance button.

    Second This. I have encountered exactly the same problem last night. (p.s. Im on beholder, so I dont think its server :) )

    Of note, I did have a crash when I first attempted to do this, and it was either during or after the patching as this was my first time for the dungeon in question (Lair of the mad dragon).
