So after my review, I was sitting here having a think and came up with an interesting concept. As noted in my review in the game play and feedback section, I am not a fan of the determination bar. I was thinking that it acts like a "rage bar" which should be a mechanic for a barbarian, not a fighter. Fighters are tactical, measured, and deliberate in their plans and actions. In this regard the following suggestion should not only capture the essence of a master tactician in game play, but also address some of my concerns about the survival that is built into the class along with adding some on demand, burst damage.
1) Change the name of the "determination" bar to "tactics"
2) The bar would work in two stages as it does now
3) Stage one (half full): All damage incoming is reduced by 5% and all damage outgoing is increased by 5%
4) Stage two (full bar): All damage incoming is reduced by 10% and all damage outgoing is increased by 10%
5) Tab toggle: Upon selecting tab you will use up 1 charge off the bar. If the bar is full it will drop to half full and you may not tab toggle into a "tactical maneuver" unless the bar is at least half full. Any amount of tactics that have been built up beyond 1 charge, if the bar is beyond half full but not yet full, will remain after the bottom half of the bar is consumed in a "tactical maneuver".
6) Tactical Maneuver (using tab): Will reset all cool downs on your encounter powers. If all encounter powers are NOT on cool down it will instead instantly fill the action points on your daily power, thereby resetting the cool down, or allowing the use of a daily power, if you previously had no action points.
Tactical points would not diminish over time. If someone in your group needs to go away because life happens, no need to worry, your tactical bar will stay right where it is. This way there is no penalty to your performance based on the actions of others. Well at least not in the mechanics of your tab action bar which is a pretty big deal.
Obviously this is just a suggestion. It is just an idea and any implementation of it that the developer chose to go with of course would be completely on their interpretation. I just thought I would throw it out there as food for thought if nothing else.

I disagree with the fluffy end of this. It's entirely possible to have a fighter that's a total berserker in combat, charging into the middle of enemies without regard for their own health and swinging until they're dead or the enemy is. (More or less how GWF plays, come to think of it.)
Plus, this is actually rather overpowered, to be honest. Do you really want to see groups of GWF's all using, say, Avalanche of Steel or Crescendo twice or 3 times in a row on some boss? The game would pretty much go the way of STO and become "Great Weapon Fighters online" with all the other classes becoming utterly redundant in comparison to the game-breaking damage that would be unleashed. (Especially Avalanche with its invulnerability period when you're using it.)
I think a tactics bar or reducing cooldowns in general is something that should be given to the Warlord class if they come out with it, reflecting that class being...Well. A leader, not just in role, but in overall nature. Picture it..."Bring it down, NOW!" It might also be a nifty way of implementing their powers that are essentially "You make someone else attack for you."

I think you would do more damage by keeping the 10% buff up than by blowing the bar to reset the cool downs. It is all about choosing steady as she goes better damage or I need it now burst damage. As for the fear of chaining 3 daily cool downs keep in mind that they could add in other mechanics if it seems too powerful. They could make it use the entire bar to reset a daily. Or they could make it put all your encounter powers on cool down if used to reset a daily. It is all about options.
To me the Berzerker and rage have always been the realm of barbarians. Right now Sword Master is the only paragon available. This conveys an image of a precise and masterful dancer with a blade, and a very big one at that. The Sword Master is not some blindly flailing, ball of rage and reckless abandon. He knows exactly which maneuver to perform to counter the opponent every time. To me he is like a fencer with a darn big sword. Or at least that is the type of image I get from it, quite far from your typical barbarian of fury if you hurt me I will hurt you worse type of mentality.
The Warlord is an interesting point and a valid one, if they don't do this bar for the GWF then they absolutely should for the Warlord. Who knows it might even make me buy a character slot and reroll my main if they pulled it off well. You have to admit, at the end of the day, the mechanic of my suggested bar is far more interesting and tactical then what the GWF has.
