Upgrading Mirage Does Nothing

As you can see the talisman isn't showing up in the box "(Empty)".
It showed up before upgrade failed a few times. Disappeared after
it said 100% chance. "Upgrade" button makes the sound effect of
starting, but does absolutely nothing at all.

Tried closing out the upgrade panel, tried putting other
items in... When I try the Mirage Talisman it goes back
to being blank, and accomplishes nothing.

Edit Notes: As I load the save point for this Toon it says
"You do not meet the requirements for this item"
Not sure if it's related, hadn't noticed it happening before.
Upgraded an Enchant afterward with no problems.
Am not going to risk a Coal Mote just to bug hunt.

Any work arounds?

    To be honest it afects also aboleth and other from the same area, also comple of others across the whole game that get stuck on lvl "green" with 100% chance to be upgraded. I lost good deal of components on different onces :(
    The question is - WHEN? it is really disappointing to grind through all the requirements for a month or longer and be unable to get the upgrade and desired artefact.
