Nostura in Spellplague Caverns does not spawn portals before the wipe

At the end of the fight you are supposed to destroy Planar Tears and go through the portals to avoid the wipe. Sometimes the portals do not appear. I have seen this bug twice on the first attempt.

    How fast did you kill her? If you take her health down too quick mechanics get skipped and it will be a team wipe. Shouldn't be like that, but it is. You have to go slow enough for all the different phases to happen. I usually just use at-wills until after the last portal phase.

    She was exactly at 20% last time it happened.

    It doesn't matter what her % is when it happens. What matters is how fast did you get her there. Nostura doesn't like speed burns.

    we had the same issue twice yesterday, both times the team was neither too high iLvl, nor exp, so it wasn't anything close to a speed burn. none of the mechanics got skipped, shooting portals / tentacles / fey realm/ 'you've doomed yourselves' followed by a wipe without even a hint of planar tears to kill to spawn portals

    You have to encounter 2 tentacle phases or it does this. When you had the issue did the tentacles appear twice? If not, she was burned too fast.

    again, went to SC intentionally today to check whether the issue persists, aand yep it really does. the party was random, not DPS-heavy, went through all the phases, as same as yesterday, except that the fey phases (the tentacle phases) were done a bit faster today.
    the only noticeable difference between the 2 days of using mass life scrolls in SC was that today she did spawn planar tears, but killing them did not spawn the portals to counteract the wipe.
    to further reiterate the fact that this isn't just a boss-burn issue (unlike Drufi) it's worth mentioning that yesterday the only people alive enough to undergo the final wipe were the paladin tank, my healer cleric and a 27k DPS. like I said, in those conditions it would be kinda hard to do anything remotely resembling a burn
