Avernus Wastes removes your companion

If you change instances and the game thinks you’re closest to the original Avernus Wastes gate you will have your companion set to null.

It’s noticeable as I get my butt kicked cutting past a band of imps and cultists.

    If a queue pops while driving your car and you go to an instance you lose your pet also.

    So, warmachines need sidecars.

    Happened to me also, if the queue pops while you are in the car sometimes the companion is unsummoned
