This new Mod and Fuse system is confusing me to no end. Is there anyone out there that can point me to a guide or even help explain this nightmare of a system better than the game did? (gotta love outdated help systems)
I'll give it a try. Let's start with fusions, not all equipment can have mods fused to it. Only specific Primary gear; Offense, Defense, and Utility. This gear will have little circles in it and it you open it up by holding control and right clicking it, it will tell you what type of mods it will accept. Scrolling over it with your mouse should also let you know if it will accept mods. Now, the different type pf mods are enhacement, armor, and "role" (I don't remember what the last one is called, but it works with offense, defense, or utility depending on the gear).
You can combine 5 of the same mod to make the next level mod. So 5 Level 1 Constitution Enhancements will make a single Level 2 Constitution Enhancement. 5 Level 1 Constitution Armor will make a single Level 2 Constitution Armor and so on. There is a certain percentage of how successful you will be while combining mods, so be aware of it.
Currently most people just get mods up to level 5 because anything more is not that big of an increase in effectiveness.

Notice that the "Gamblers Lucky Gem" is the most useful of Core Mods and is handy to have in any of the 3 Primary Upgrades. Lucky gems are consequently a bit more expensive than other core mods.
The new system has problems but does allow us to make up our own gear with the stats that we want on it.
The best gear you can get has 3 slots available (at level 40)... These are..
Silver Recog or perhaps Questionite gear
Primary Offence 1 x Core and 2 x Enhancement Slots
Primary Defence 1 x Core and 2 x Armor Slots
Primary Utility 1 x Core, 1 x Enhancement and 1 x Armor Slots
Blue level gear (level 35 ish to level 40)
Primary Offence 1 x Core and 1 x Enhancement
Primary Defence 1 x Core and 1 x Armor
Primary Utility 1 x Core and 1 x Armor or Enhancement not both.
Drop rate and stuff
It seems that rank 4 and 5 mods may drop from any mob but are more inclined to drop from late game mobs. Rank 5 mods are unusual and Rank 4 mods are the norm.
Level 40 blue primaries drop from level 40 mobs except for Utilities, Primary Utilities that drop from mobs are always green. Some of the quest rewards are blue primary utilities but it is really hard to get a level 40 blue primary utility.
Silver recognition can be got from doing Unity or Unity 2 Quest of Elements missions. 18 recog for each set of missions you do. Silver Recognition Primaries cost 35 Silver Recog each and are the best easily available gear.
Tip when equipping your character experiment with blue and green primaries and Rank 4 mods maximum, save your rank 5 mods for the Silver recog or Questionite gear which you will likely grind for.
Early game gear has at most 1 modifiable slot this will almost certainly be a Core Slot. The good stuff seems to start at MI.
