Can anyone please answer me, when exactly this advantage is triggered?
Formerly it was simple, Enrage was a click power and you got the Heal when you activated it.
Now Enrage is a Form and Wiki is still outdated about it.
What triggers the advantage?
Gaining a stack of Enrage. In other words, when you use a power that deals knockback.
Keep in mind that you can only get a stack of Enrage every few seconds, even if you're spamming knockback powers galore. I can't remember if it's 3 or 4 seconds between each stack.

Funny thing about it is, I took the power in advance at the moment.^^
Right now, I don't have any power with knockback (next level I start picking KB powers), but it activates anyways every now and than, without any pattern I understand.
Activates in what way? At rank 1 of any Form (Enrage included), it will maintain one stack of the ability. That may be where it's coming from. o.o Another possibility is that your Energy Builder has a knockback function that only triggers at certain percentage of the time. Sonic Blaster, Force Bolts, Boomerang Toss, etc.

Activates in a healing way.
Animation, sound, HoT and of course the sign under my status shows me that Endorphin Rush is active.
My only attacks at the moment are Clobber, Def Combo and Mighty Leap(w/o advantage).
If it also triggers with every stack I get, fine, but I just wanted to know what it triggers it right now?
Some timer?
Nope...trust me...I have no additional power atm that give me extra stacks of enrage.
I'm just Lvl 13...I created this toon tonight.
Beside the attacks I already mentioned, I have Defiance running...nothing else.
I'm glad about every bit of health dropping down on me...but I just want to know.

Not sure, then. XD Clobber refreshes stacks of Enrage when you take the Advantage, but my testing concluded that refreshing them doesn't trigger Endorphine Rush. So, I have no clue!

If I'm not mistaken, the toggle powers/forms don't just give you one permanent stack of the power. Instead, it's on a constantly refreshing timer. Like, 15 seconds of Enraged, refreshing every 15 seconds. Could be wrong, but I'm pretty sure that the icons under your character portrait show one for the form just being on, and then another icon right next to it that represents your stacks (which has the timer on it). Pretty sure that's even there when you're totally at rest. Not at my comp though, could definitely be wrong!
