So we're supposed to defeat 15 Minions before we can go inside, right?
Since last week's patch, there are only 13 in the Nemesis version of the Bank Robbery. That means that the team defeats those 13, and then can't go inside because the other two we need to defeat just aren't there.
With other foes, there are the usual number (which is, what, 20? 18?). As far as I can tell, this only affects the Nemesis version.
This seems to happen with certain minion types and not others, the Thug minions in particular. If it is affecting your Nemesis, try changing minion type, see if that fixes it.
Nemesis system is one big pile of bugs and it's getting worse with every month. It really needs to be finally fixed.

Not only they are vanishing or dying suddenly (not to mention that one of my toons is unable to get any clues from minions... for more than three months, and changing minion types and powers fixed nothing), but now they are sometimes invisible in nemesis editor (can change their type and powers, but models are invisible).
This system really should be fixed. It doesn't do any good with players retention.

I'd played this at least once before, since the above post, and thought it was fixed. No such luck.
