I Added Implosion Engine at the End, because he gave me the option to do so...and because it sounds like an awesome final ability if you wanna do some shock and awe.
You can also swap out the Invulnerability for any Defensive Passive such as Regen, or LR...Just Gear Accordingly. You can also do a build utilizing LR, Sword Cyclone, Force Shield(fs), FotT BCR(rr) & High Crit-chance... then add in several heals and Defenses and you're good to go.

Maybe you just assidentally took the wrong power or maybe you just didn't know... or maybe I myself don't understand something here. But enrage is a form now, as inertial dampening field is. So you can't use both at the same time.
Also, I heard somewhere that now, when enrage is a form and doesn't work the way it worked before, invinsible is a better option then defiance. Can't give you any proves, it's just something I heard.

gear for critstrike and cooldown and you shouldn't have much of a problem with energy..
Also, try to spam-tap annihilate when you can.
One problem you could run into IS energy management when you do spam-tap annihilate...
I currently use a 'bugged' healing tank build that uses certain bugged aspects to regen hp at an alarming rate (damn cryptic breaking stuff and making me want to use said broken things..)
Honestly I don't like enrage anymore...I miss the old enrage...I love compassion now with a...'passion' but right now your build isn't lacking in any department...you just have 1 too many forms and no idea what you want to be (a tank or a tank-y dps) The build I provided makes you into a tank with a medium base defense (since you have defiance you don't need the high defense that a regen/LR tank will use)
Grab yourself some dodge/avoidance gear where the avoidance comes from silver champ gear and the dodge from rank 7-9 gambler.
All in all, I don't care for pvp so don't use the build I have provided for it as I have never pvp'd and will most likely only test out holds on players(hopefully my 2 min holds aren't 2 mins on players.....right...?)
Enrage may have to be swapped out for IDF because IDF was the bubble power that kept you from taking so much damage... You could basically Turtle defense yourself while healing, and then jump back into action with Annihilate and other abilities.
So i'll re-imagine your build and add in IDF

Crashing Wave Kick with Subtlety of the Tides is a very potent dodge-stacker, and should help beef up your damage reduction considerably if you remember to tap it every once in a while.
Eye of the Storm uses dual-swords, which isn't exactly heavy weaponry, but you can choose costume pieces for it that look heavy! It's a decent AoE attack for damage, but it really shines in the shielding it provides. That shield can absorb darn near everything. XD
Devour Essence doesn't really match with your theme, sadly, but it puts out some very respectable damage, heals you as you do so, and with Supernatural Power, you don't have to worry about running out of energy at aaall. XD
I'd actually possibly switch out Form of the Tempest for Compassion, instead, as it increases the healing you get from Devour Essence and Bountiful Chi Resurgence. As far as I'm aware, anyway.
Inertial Dampening Field can't be used alongside other Forms anymore, so ignore that. XD
Defensive Combo gives an extra little stack of Defiance on top of the resistance you get from Invulnerability and your items. Plus, major threat generation!
The build was able to put out "decent enough" damage thanks to Form of the Tempest and the critical hits granted by Dexterity. Going Compassion (and taking Presence or Recovery as a Superstat to boost it, of course) would sadly end up reducing that a bit. It's a bit of a tradeoff between damage and survivability, there. Form of the Tempest for more damage, Compassion for staying alive better.
I'm not sure how effective Bountiful Chi Resurgence with Resurgent Reiki is, anymore. I hear it got a cooldown between how often the dodge-based healing could happen. I'm also not sure if it actually triggers twice per dodge when you have it stacked, now. It's been quite a while. XD
There's no AoE threat-generating power in that build, either. You'll likely want one of those.
Seriously, though, Crashing Wave Kick. Adore that power.

So I may try to figure something out with this build of yours... trying to modernize it will be difficult .. as in not really knowing what 3rd super stat to get, what Specs to use.... what gear to try.
I mean the Invuln and Defiance stack will make you an armored shell... same as Invuln with Field Surge, which is not a form either but a 1,500K bubble which lasts 15 seconds.. long enough to heal up or other.
We will get this figured out soon enough.
First thing you should understand is that one cannot excel at both. My current FF pvper Charlize does fine for pve, but she's far from pve efficient.
Looking at your initial build, you either have really really easy competition, or haven't pvp'd against any of the regulars post-alert.
FG/Hurl are your only real nttg carrier options. Lunge is far too unreliable, let alone lacking in distance.
Melee in pvp are extremely far behind compared to where they stood pre-alert. Annihilate won't kill anyone without lots of help or you catch them fighting someone else and having a sliver left.
IDF is just shy of meaningless in pvp - most hits are well over 1k if not 5-10k, making idf's minor flat mitigation pointless and an energy drain instead of having an energy producing toggle like enrage.
If you have any melee 40s, AT or FF - take them into bash for a few rounds, and most likely one of the regulars will be around to show you firsthand just how painful melee is. Once your MD is down, your defenses generally won't last long.
But if i got to choose... then PVE Elite solo style build is what I would like, do you have any Ideas?
Depends on what you mean by 'PVE Elite solo' If your talking about 'elite' difficulty then just be a dps with ads and self heals and perception debuffs
If your talking about taking and surviving ungodly amounts of damage then I can help you there..
I can share an infernal build that can utilize any defensive passive and still survive most attacks(Though I have never tried out PFF with it but since it is -almost- like regen it should work fairly well minus the ability to regen 1.4k hp every 3 seconds)

Bind a key to spam devour essence and use both, block and your ads when you feel the need to and there shouldn't be any problems whatsoever.
Invul is just a placeholder, you can pick any single defense passive to use and you will survive easily(except maybe pff...didn't test it out with it) and the last power is there for your choosing...but honestly, you won't have the sheer brute force that a dps does...well....if you spam massacre you will...but it requires a bit too much recovery..
Though in all honesty if you really want a quick PvE Farm build then all you really need is:
LR Sword Cyclone ForceShield(fs) FotT BCR(rr) High Crit-chance and at least 1 mob(alive) attacking you
After all this time good ol' LRFSBCRFOTTSC still powers through all content(except gravitar...sadly..)
Epidemic, and Palliate have to be moved farther down.

That said... normal PVE is wowzers excellent... I did a mission 2 levels higher than me and cut thru it quickly... So the build up top is viable for Missions, and normal mobs... On Alert, you need to watch it.

Level 6: Constitution (Primary) Obviously, you're going to want HP as a tank. This also gives you the Constitution Specialization tree, which-... Well, it's not a favorite of mine, but it has a few decent-enough defensive options. XD
Level 10: Presence (Secondary) Presence boosts your healing ability straight-up, and makes Compassion more effective. You could replace this with Recovery if Energy ends up being an issue, as Compassion scales with that stat as well.
Level 15: Dexterity (Secondary) This boosts the dodge and avoidance given by Parry's Elusive Monk advantage, as well as making you get criticals more often. And, if you choose to grab one of the Martial Arts forms, it will boost the damage buff from that, as well.
Talents: Just maximizing your Superstats. No fancy off-statting here!

Level 1: Vicious Strikes Usually, choosing an Energy Builder isn't that big of an issue. However, Vicious Strikes differentiates itself from other Energy Builders by way of having the fastest attack speed out of all of them. Just 0.3 seconds per attack! You still aren't going to be using your Energy Builder all that much, but that attack speed paired with Crashing Wave Kick's Subtlety of the Tides advantage means that your dodge and avoidance will be sitting pretty.
Level 1: Crashing Wave Kick (Subtlety of the Tides) The advantage on this is awesome. Just tap it every once in awhile for a free chunk of dodge and avoidance that will really send your survivability skyrocketing.
Level 6: Parry (The Elusive Monk) Gives another boost of dodge and avoidance just for using melee attacks!
Level 8: Devour Essence (Rank 2, Rank 3) Nice damage, and returns health to you. Paired with Compassion below, the health regeneration will make sure that the little damage you DO take means absolutely nothing. If you really want to, you can replace Rank 3 with Crippling Challenge, instead, which will let you more effectively keep the enemy on you while still staying alive. It also hits once every 0.5 seconds, meaning you'll be triggering Subtlety of the Tides very often.
Level 11: Supernatural Power This makes it possible to just keep using Devour Essence for eternity. It's incredibly effective even without Recovery, but if you do decide to replace Presence for Recovery, you won't even remember you have an energy bar.
Level 14: Invulnerability (Rank 2, Rank 3) Most forms of defense just reduce damage at a percentage, rather than taking off a chunk of damage. Invulnerability, with its flat damage reduction, makes it so that even the largest groups of small-game enemies mean nothing at all to you. You have plenty of other percentage-based defenses, so you want this to cover all your bases.
Level 17: Compassion This boosts up your healing from Devour Essence and Bountiful Chi Resurgence (further into the build), and gives a little bit of a damage boost, as well. Shouldn't be at all difficult to keep up, given that you'll be healing from Devour Essence and Bountiful Chi Resurgence, as well as Constitution's "Adrenaline Rush" Specialization any time you get a critical hit. If you so choose, you can drop one rank of Adrenaline Rush in exchange for one rank of Quick Healing, which gives you a constant stream of (very small) heals that will make it impossible not to have a full stack of Compassion unless you spend a long time outside of combat.
Level 20: Eye of the Storm (Rank 2, Rank 3) This does decent-enough damage all around you if you just press the button in short bursts, so that you get the highest damage possible out of it. And, if you keep the button pressed, the shielding you receive from it will get more and more effective, until pretty much nothing but the biggest bosses can hurt you through it! If you wish, you can drop Rank 3 and take Blade Beyond the Veil, instead, which does a bit of damage to enemies attacking you in melee range. This can actually clear out enemies pretty well, but Rank 3 is a bit more reliable and gives you a bigger shield. Lastly, Eye of the Storm has a 0.5 second delay between each tick of damage, which means lots of chances for Subtlety of the Tides to kick in!
Level 23: Defensive Combo (Rank 2, Crippling Challenge) This is the main threat generator against bosses, and hitting three times in a row with it will also grant one stack of Defiance, boosting your defense up just a little more. Crippling Challenge will also reduce the damage the enemy does, thus making you take even less damage!
Level 26: Support Drones (Rank 2, Rank 3) Little bit of extra healing that doesn't require attention. Just plop them down and forget about them! The only problem is that their healing doesn't give you stacks of Compassion, so if you're reducing so much damage that the Support Drones alone are keeping you at full health, then you aren't going to be able to maintain Compassion. But, if you're staying alive that well, you obviously don't need the bonus healing!
Level 29: Bountiful Chi Resurgence (Resurgent Reiki) This gives you yet more healing, and tosses you another heal every single time you dodge an attack, which should be very often with this build! This basically makes it so that fighting large groups of small enemies heals you instead of hurting you.
Level 32: Shuriken Storm (Challenging Strikes) This is honestly just tacked on as an AoE attack that will trigger Challenging Strikes on enemies, bringing them all to you instead of your allies. It has a chance to miss enemies and not deal damage, but from what I've read, it will still apply Challenging Strikes regardless. You could also take the Floating Butterfly advantage to give you another boost to dodge, but by that point, it wouldn't be giving much of an effect due to how many sources of dodge you already have. You could honestly replace this with something else if you don't mind losing out on a group threat-generator.
Level 35: Redemption This is another tacked-on power, which takes advantage of the fact that you'll pretty much never die. Unfortunately, taking damage cancels the power, which means you'll have to get rid of the enemy's attention somehow. If you've been using Defensive Combo on it, that'll be difficult. Teleport might help with that, but once you reappear, it's possible they'll just chase you right back down. Redemption's "Salvation" advantage is nice to have, making it a 50-foot revival power that can work on multiple allies at once. Not a necessity, but definitely useful!
Level 38: Form of the Tempest Most people despise taking more than one Form (or Slotted Passive, or whatever else that can only be used one-at-a-time), but I think it could work for you, here. If it turns out that you're surviving just fine without Compassion's healing boost, you can switch to this and do more damage, instead! This particular Form grants a stack of Focus each time you get a critical, which you should be doing often with Dexterity as a Superstat. You could, alternatively, go for Form of the Master, which triggers each time you dodge an attack. Form of the Master also has Storm's Eye Prana as an advantage, which cuts the cooldown time of your self-healing abilities each time you dodge an attack. This means two Bountiful Chi Resurgence buffs at a time! But, getting stacks of Focus from dodging instead of from critical-ing is a bit less reliable, so I personally prefer Form of the Tempest. Besides, you have Compassion for survivability purposes!
Travel Powers:
Level 6: Teleportation For those rare times when you start taking more damage than you can heal back. This lets you get out of the battle in a hurry, becoming invisible to the enemy.
Level 35: Athletics (Rank 2) Since you have no lunges or roots, this is what keeps you on the enemy with your melee attacks. You can toss it up to Rank 3 for even more speed, but I've generally found Rank 2 to be sufficient. You could also take the Versatility advantage, which boosts your movement up when you take damage. With Rank 3 and Versatility, you may actually be moving so fast in combat that you'll have trouble controlling yourself.
Specializations:
Constitution: Unyielding (2/2) Just a bit of a boost to keep yourself from being stuck in place.
Constitution: Fuel My Fire (1/3) Little more energy whenever you get hit. Not a huge boost, but somewhat useful.
Constitution: Resilient (2/2) Knocks are aggravating. This makes them less so!
Constitution: Deflection (3/3) Yet another boost to your dodge rating.
Constitution: Adrenaline Rush (2/2) You're going to be getting a decent amount of criticals, and this heals you for a percentage of your HP when you get one. Should help with keeping you topped off!
Vindicator: Aggressive Stance (2/2) This will play a big part in boosting up your damage resistance. Paired with The Best Defense from Warden, you'll create a feedback loop between your Offense and Defense that makes both stats fly high. Offense isn't terribly useful in large quantities, but Defense helps with every single point you load into it!
Vindicator: Merciless (3/3) This one isn't defensive, but will help you deal more damage, even if it's a secondary function of the build.
Vindicator: The Rush of Battle (3/3) This heals you for a nice, big percentage of your total HP when you've recently killed off an enemy. Doesn't heal you for every enemy killed, but it's still a big help.
Vindicator: Modified Gear (2/2) Just boosts up the effectiveness of that feedback loop mentioned above.
Warden: Fortified Gear (3/3) More help for the feedback loop.
Warden: Ruthless (2/2) More help for your damage output.
Warden: Tenacious (2/2) More help for the feedback loop, takes effect when you get hit. Should be quite often!
Warden: The Best Defense (3/3) The other half of the feedback loop. Will make sure your Defense stat gets nice and high!
Mastery: Constitution Mastery (1/1) Not a huge boost, but better than nothing!
This leaves you with a few Advantage Points that you can throw around willy-nilly.
As for gear, you'll want to keep it balanced. I would recommend...
Heroic Breastplate of Agility + Gambler's Lucky Gem mod. Boosts your normal (not in-combat) dodge and avoidance.
Heroic Gloves of Offense + Impact Gem mod. Boosts your Offense, which helps with the feedback loop, giving you more Defense stat.
Heroic Helmet of Energy + Growth Amulet mod. Truth be told, any of the Utility primaries or mods will be about as useful as the next. You shouldn't be hurting for energy, but these two will roundaboutly bring up the threshold for when Supernatural Power will trigger, by boosting your maximum Energy.
Vigilante's ____ Belt. Any of them will do, really. Critical helps you deal more damage, Rejuvenation means a little more healing from Devour Essence and Bountiful Chi Resurgence. (And possibly more shield strength on Eye of the Storm, too? I'm not sure if that's boosted by healing bonuses.) And Impacting means higher Offense, which means higher Defense. Impacting doesn't have a heck of a lot more Offense than the other choices, though, so I'm not sure it's that helpful.
Vigilante's ____ Bracers. Again, either one will help, really. One gives a tiny, tiny bit more Defense, and one gives a small health bonus. I'd personally go for the health, but it's up to you.
Vigilante's Hastened Eyepiece. Bit of a health boost, you won't need the Cost Discount from the other one.
As for which stat mods to take, just do three each of Dexterity and Presence. The Vigilante gear will take care of your Constitution.
Assuming you get Rank 5 stat mods (fairly easy to manage), you should have 262 Constitution, 240 Presence, and 233 Dexterity. You could use a bit more critical chance, so you might want to sacrifice a little bit of Defense by replacing the Impact Gem on your Primary Offense with Gambler's Lucky Gem to get more Critical Strike. You could also opt for the Heroic Gloves of Precision instead of Heroic Gloves of Offense for a bit more critical chance. That's probably a better choice than changing the mod, given that you'll get Critical Strike while still having some Offense added.

I will keep the other one up there since it does great for farming and a great Soul Siphon theme for someone looking for that kind of thing. But i'm also keeping yours right under that for a secondary more offensive approach to the game.... Soul Siphon build is great as a Support toon, you sit in the back and debuff everyone and wipe the henchmen while your other members go after the big names.

Bountiful Chi Resurgence should trigger Compassion's energy gains once you get it, too. That'll help out.

Chimera went down... Rhino ... not too long after... That Revive worked nicely because the lvl 40 was DPS and agro'd more than I can keep... and I ended up reviving him a few times... the revive also came in handy during Gravitar... though This build (if not any build) is difficult against her.
Ao'Quephoth ... though i was not able to kill him, he was unable to damage me as well.
I've seen some people with tank characters saying that Gravitar is easy for them. I'm not sure what their builds are, though, sadly.

Problem... i dunno if having FoTT is much use, the damage increase is not much.. so I was thinking about changing the last power to a more powerful heal, like resurgence, or Palliate if i'm in a group. for those, Just in case moments where I'm a little low on that green bar.
or what do you think about Unbreakable?
Also those drones... they die so quickly ... possibly within seconds of the first mob agro... i may have to swap that with a better heal to power Compassion.
Its dumb that you cannot Dodge or absorb Falling damage .. you take the full blunt of it every time.
Form of the Tempest is just there for if you decide to deal damage instead of tank (though you'll still be quite durable, and you still won't deal as much as a dedicated damage dealer). If you want to put more focus on the tanking aspect, then yeah, take something else! Resurgence and Palliate both have really high cooldowns, but if you're just after a quick "emergency healing" option, then they should do fine.
That said, as far as the damage boost from Form of the Tempest, it should be fairly decent, around 10.5% damage per stack. What is your Dexterity at?
Unbreakable is helpful against small enemies with lots of tiny hits (much like Invulnerability), but it doesn't do a heck of a lot against bigger attacks like the ones Gravitar dishes out, and to my knowledge, bigger hits may actually "deactivate" Unbreakable by taking out its shield in one shot, after which it won't regenerate the shield and thus stops affecting you. I'd take Masterful Dodge instead, if either of them. But, I'm not a big fan of the Active Offense/Defense powers, given their long cooldowns. XD
If you're liking for different powers to put in instead of Form of the Tempest, one relatively "fire-and-forget"-style power you could try out is Sigils of Ebon Weakness. This tacks yet another level of defense onto the character, one that affects the entire team, by reducing the damage that enemies deal in the first place. It's not all that great when you're moving around a lot, but against relatively-stationary enemies (such as Gravitar), it can make a big difference.
Unfortunately, her AoE attacks may end up taking out the Sigils over time, but if you summon them while you're right next to her, they'll appear on all sides, and she'll have to fire multiple Force Cascades to get rid of them.
The Support Drones aren't going to hold up well against Gravitar due to her plethora of AoE attacks, no. When you're up against enemies that attack one thing at a time, though, they're a big help!
If you're often fighting things that attack in an area, and likewise not fighting things that only attack at you alone, then yeah, you might want to drop Support Drones for something else.
For Gravitar specifically, you may actually like Circle of Primal Dominion. You need to stay in one place, which isn't generally a good thing for a melee tank character, but if your opponent is staying still for long periods of time, it can be useful. Not only does it heal you and make you harder to knock around, it also increases your Threat generation!
As for falling damage, is it just knockback, or are you Travel Powers being taken out, as well? If you have Athletics/Acrobatics active, it should reduce the falling damage you take a little bit. You could, alternatively, take a Flight- or Superjump-based power. Flight will make you come to a halt mid-air after knockback, and Superjump cuts the amount of damage you take from falling damage by a massive amount. I think it may even nullify fall damage completely at Rank 3.
One more Travel Power option is Swinging. It doesn't reduce fall damage as much as the other two above, but you can take its Flippin' advantage for a minor boost to your dodge rate, which will help your defense just a teensy bit more.
If you're looking to replace those few powers above, I'd recommend Sigils of Ebon Weakness as already stated, Conviction, or Bionic Shielding. You don't want to take a power that requires a lot of attention, like Empathic Healing or Celestial Conduit, since that cuts into time that could be spent using Devour Essence. They may heal more, but they nullify the point of having taken Devour Essence in the first place, and result in putting out less damage yourself. With the other three listed above, you just tap the power and keep on fighting.

One thing I noticed during my testings is that gravity cuts through everything, even immunity to all damage oh and Swinging/Super Jump/jetboots negate almost all pve fall damage without you ever having to invest any points in them.
Gravity isn't your friend but it doesn't hate you...

I can honestly say this: do NOT use the build above if you want to solo things.
This build will make you into a tanking healer. You cannot hold aggro if you are spamming iniquity so there are certain problems with threat that you will constantly run into so be ready for that.
The last power can be anything you choose though, in all honesty, I cannot find any useful power(that fits into the type of 'role' it was made for) to fill the last slot...
I will not share what kind of gear you should be looking into, instead, let me give you a run quick summary of what it aims to be.
(1)You can either run up to the boss and tank it by spamming devour essence alone(and never lose threat)
(2)You can either run up to the boss and tank it by spamming devour essence then switch to iniquity spam on your teammates to heal them up because they took at hit or two from aoes or the mechanic of the fight in question and continue spamming devour essence and iniquity whenever you feel the need to.
(3)You can run up to the boss and start out by spamming iniquity, letting the other tank take aggro while you keep all your teammates alive. When the boss one-shots a person you can rez them and continue your iniquity spam until the fight is over.
(Note: Iniquity has a 100ft range so as long as your teammates aren't AFK or using a 120ft power, you can always heal them while you are sucking face with the boss)
(Other Note: This build uses and abuses the bugged pre tree so when they fix it you won't be as...'well off' with iniquity spam)
I do not know if iniquity makes your teammates stop using their block...though if your healing them then I don't really see much of a problem if it did..
Edit: Incase a dev or someone who is wondering what exactly is 'bugged' about this build let me just spit it out.
Selfless Ally heals for more than it says(might also be due to the fact of how I built myself)
Compassion raises the hp-regen of Resurgence which SHOULD NOT HAPPEN(unless the tooltip is wrong..I hope it is...since I have 11k hp and it heals me for 10k...and when I am sidekicking to 30 I get 7k hp and it heals for 9k...)
invul/dodge+idf+ebon void+adv +ebon sigils, that on top of other mitigation stuff, such as tk eruption, evasive manuevers, defensive combo, and throw in masterful dodge and/or unbreakable... and you're pretty much good to go (looks abit over the top, but that would be also refreshing protection field with electric sheath+adv with all mentioned above )

This is the tank build I run. To be completely honest, I don't even remember the last time I died in pve with this build.
I have been the last person standing on Gravitar multiple times and still completed it...although I haven't ran it recently if she got a buff or whatnot since her alert first started.
I geared: Offense-crit, Defense-HP, Utility-cost reduction...and of course, run in Tank role.
