It seems that there are either nodes spawning that can not be looted by myself or others around, or someone is looting the node then does not take all then it locks that node from everyone else.
When its a bad node, it still does the aoe explosion animation, but if you click take/take all nothing happens,

I've had this problem a few times but it usually fixes itself. Try looting it again and pressing Z instead of take all, then try again and hit take all. Seems to make it work for me
Yeah, if you even so much as twitch while looting, you won't be able to take the items, will have to close the window and loot it again.
I haven't noticed broked nodes so much as hidden/can't find where the spawned/radar not showing them properly. Like, I've seen (well guessed) a node spawn inside a cliff before or at least, that's the only explanation I have because there wasn't any scenery etc in the way. As far as I can tell, nodes are static, they always respawn in the exact same spots. My theory is that the game/radar isn't updating properly. Maybe someone looted the node recently but the game refuses to acknowledge that.
Also, while the nodes are static, they are not always displayed in the right spot on the radar or may be under scenery. For instance, while in a duo with my BF's character we were looking for his mysticism node which was right by where we were questing. He was literally standing centered where the radar said it should be, but it wasn't there. Because I like picking up objects and tossing them, I started lifting the cars out of the area and tossing them out of our way. We found his skull/book three cars down from where the radar said it was.
From what I've witnessed, either by doing it myself or viewing it, the broken nodes are actually looted nodes where the graphic hadn't disappeared. You'll notice that after its normal respawn time it'll be lootable again. Sometimes it'll actually disappear after looting it or the image will stay after you've looted it.
