Figured I'd post here. I notice Fiery Form randomly turns itself on when not in combat. I was just standing in Millenium City near the night club and Fiery Form turned on.
The countdown timer on it is also bugged, it bounces back and forth between 7-8 seconds.
***Just tested it, Fiery Form turns on anytime you get near an object you can pick up such as a street lamp or trash can, it also deals fire damage to the object (neat touch).
