bug with most "OV" offensive gear and "cosmic strike"

ive seen multiple instances of your OV/cosmic strike stacks resetting. ive personally seen this happen at Slug (QWZ OM Boss) aswell as Eido (QWZ cosmic) and in Therakiels Temple (Endgame Lair) but other players have reported this issue happening in other places aswell.

side note: this bug is more detrimental to some more then others, but the bottom line is it should be fixed.
