I've been playing CO since it went F2P and quickly became a life-timer. I was ecstatic when I discovered I could use my 360 controller and have used the same setup for years which was the default the first time I plugged in my controller:
RB = next target
LT + RB = prev target
RT + RB = switch between hostile/friendly targetting
LB + RB = clear target
LB + move RStick = switch target to next in that direction
A or B = Jump or if Flying Move up
RT + A or RT+B = Down
LStick B = travel power 1
RT + LStick B = travel power 2
RStick B = Zoom (Press again to cycle through 3 levels of Zoom)
RT + LT = Block
X or Y or B = powers 0, 1, 2
LT + X or Y or B or A = Powers 3, 4, 5, 6
LB + X or Y or B or A = Powers 7, 8, 9, 10
RT + X or Y or B = Powers 11, 12, 13
DPad_Right + X or Y or B or A or RB = Devices 0, 1, 2, 3, 4
DPad_Down + X or Y or B or A = switch build to Offensive, Defensive, Balanced, Support
DPad_Up = Interact (With Keyboard/Mouse controls it is known as hitting the "Z" key on Keyboard)
DPad_Left = Not mapped to a known function
Dpad_Left + X or Y or B or RB = Not mapped to a known function.
Suddenly, this past month the entire scheme changed (RB & LB change camera distance, have to hold trigger to look around with left stick, etc...)
I've tried every combination of control scheme and key bind set and even reset to defaults but nothing has been able to restore the above mapping.
Am I doomed to have to manually map all the key bindings? Or is there a simple fix I'm missing? The controller itself still works fine in other games.
Note, I've run this game on at 5 different computers with Win 7 and Win 10 and it's always had the scame mapping without me doing anything.

Run the command /unbind_all
Then open Options and click on Controller. Make sure "Joysticks" is set to Off and "Xbox Gamepads" is set to On.
Click on Controls and click Reset to Defaults. Make sure Mmo Controls, Champions Default, Free camera and On move are all selected for the first four options.
Then click on Key Binds and click Reset to Defaults.
Then click on HUD and click Reset to Defaults.
This should now be the controller settings for a fresh install. Test.
If that didn't do the trick, then exit the game and copy all of the following into a text file and save it in your Live folder as "xbdefaults.txt":
