Qliphotic Warzone Creepies (Creepies 1 through 4)
There are a total of 9 spawn locations in Qliphotic Warzone that we know of.There are a total of 4 Creepies that spawn in QWZ that we know of, from most common to most rare: Cheshire, Teddy Bear, Fenny Doll, Sammie.
All 4 of them can spawn in any of the 9 spawn locations and correspond to the following ????s in the perk checklist:

Millennium City Powerhouse Theater Creepie 6
(credit to @carrionbaggage for the theory; credit to @kamokami, @retehi#8400, @pantagruel01, @jedaithe, and the haberdashery hero himself @bringmeaslab for farming up the trigger item)The Teddy Bear creepy in the Qliphotic Warzone has a curious sense of fashion:
Sporting the latest in voodoo vanity, the Teddy Bear is modeling a Sovereign Sons Top Hat, obtainable by farming enemies of the Sovereign Sons faction.
With the hat in your inventory, head to the Millennium City Powerhouse Theater. In the dressing room area in back, there is a mysterious teddy bear on the ground.
If you have already found the Teddy Bear creepy in the QZone and are carrying the hat, you will get a prompt to interact with the bear: "Place your Sovereign Sons Hat on the teddy bear?" Upon doing so, the bear comes to life, donning its new headwear and saying, "Tip of the hat to you!"
We can't have bears coming to life without repercussions, so welcome it to this mortal coil with a healthy dose of violence.
The teddy will complain "Why do you hurt me?" and display some battle scars, but keep going. After enough damage is dealt, it will transform into a Creepy.
You'll receive credit for Creepy number 6 and the Upstaged perk.
The hat remains intact in your inventory.

Vibora Bay: Creepie 7
(Credit to @carrionbaggage , @bringmeaslab , and @glass-rose)1) Before starting the hunt for this Creepie you will need the following items in your inventory/personal account bank:
-Special Edition Action Figure: Fenny Plush Toy (acquired during the Winter Event from Clarence in Attack of Misfits Toy OM. The AF must be physically in your inventory, not bound nor summoned.)
- Anniversary Soda and Anniversary Cake (acquired from Anniversary Gift's you obtain during Anniversary Event. Rich and Sweet Cakes don't work
- Millenium City Hot Dog (acquired from Hot Dog Vendors in MC's Downtown)
2) Head over to Carl's Gym in Millenium City. The hint that leads you here is in Vibora Bay Arcade's scoreboard game. From it you can extract 2062 Fenny, 640 Toys, 60 Likes. If you reorder them to form the message "Fenny Likes Toys" you get the coordinates 2062, 60, 640 which leads you to Carl's Gym
3) Once inside go to the central room with the Box Ring. Look inside the Laundry Carts to the left and inside one of them you will find a Fenny Toy. Interacting with it will give you the perk Strange Encounter which will tell you to "Show Fenny that you are a fan".
4) Head back to Fenny in Vibora Bay Arcade. A prompt will appear to "Cheer Fenny?". Selecting it will activate Fenny. (Special Edition Action Figure: Fenny Plush Toy is required for this but will not be consumed in the process)
5) Once Fenny is active, a second prompt will appear to "Feed Fenny?". Selecting it will consume Millenium City Hot Dog, Anniversary Soda and Anniversary Cake. After this Fenny will roar, knock you back, and you will receive the perk That Could Have Gone Better

Vibora Bay/Qliphotic Warzone: Creepie 13
(credit: @pantagruel01)- First you must find one of Sammie's Dolls scattered throughout Vibora Bay and defeat them. Upon defeat, the dolls have a chance to drop an Arcade Token
Sammie's Lost Dollies Spawn Locations:
(Note: Only one doll spawns at a time, and if not engaged, despawns after about 2 minutes. After despawn or being killed the next doll will spawn after 5-10 minutes)
- Once you get the token, head to Vibora Bay's Arcade and activate the Arcade Machine. You will get an error message with the words "QLIW 1713 1133 4029" and the Nope Nope Nope perk.
- Head to the 1713 1133 4029 coordinate in the Qliphotic Warzone map (immediately to the east of Shadowfall, use /loc command to find the right place). You must be outside of party and completely alone in the area. The 13th Creepie will spawn and you will receive the Concerned perk. (note: if the Creepie does not spawn, try killing the Chaotic Imps nearby and waiting a few seconds).
