If the Defeated dialogue is dismissed without making a selection, and that is very easy to do by accident when playing with a gamepad, it's impossible to do anything except leave the game unless you have a self revive ready to go. That should be fixed.
A related issue, there needs to be either a short timer before you can make a selection after being defeated or the controller binding on that dialogue box needs to be removed. It's very easy to make a selection by accident before you even know you're defeated.
