This exact thing has just happened to me! Got the other items but no eyes of the wicked. Couldn't figure out how to raise a ticket about it. They don't make it easy on PS4 to report problems like this!

I logged on to my main for the first time since the new mod and had to do a respec. I have 2 loadouts for my rogue, one for each paragon path. I did the respec on one, then switched loadouts to respec the other. When I then switched back to loadout I did first it had reset again! Tried it twice now and it keeps resetting.…

My rogues inventory is full of weapons I want to add to library, but can't. Any idea when the patch is coming to fix this problem? The bug is bugging me now.
