That happened once to me (as a cleric I guess I don't usually get the best score) but it wasn't for the item anyway. The leader was just mad for some reason and start kicking people... oh well, pvp needs a lot of fixes and additions after all, hopefully we will see them some time soon.

I haven't expirience the first part but for the pvp issue, although they should fix it, you can easily wait a bit longer in the pvp map untill you respawn there, then go back so you don't have to respawn in the city and use any injury kits.
