Yes. Very Annoying. This happens when switching between different gear pieces without closing the window and leaving the dye in the slot.
Say on the first item you are looking at it in the preview with the dye on the primary. You switch to another piece of gear. Without removing the dye or closing the window. It shows you the dye on primary still (but on the new item). But if you use the dye, it dyes the accent slot (iirc). Since that is the one that would have been on if you had closed the window and started over instead.
You can avoid it by clicking a different part (accent, primary, secondary) and then clicking back to the one you want to dye.
