always getting the same item from every reward chest

I helped a lower level friend with some of his quests and he shared them with me. Now every time I complete a (Foundry) quest, dungeon or instance, no matter which one, there is the same unique item in the reward chest that I cannot take because I already got it. Some rare cloak. That even prevents me from successfully finishing quests and dungeons. I had to abandon 2 finished Foundry quests and one regular quest because I couldn't end them without taking the reward.
If this isn't fixed I might never again be able to normally end a dungeon or quest!
