When i get to fight against a lot of mobs i get extreme lags. This happens most of the times in a 5 man dungeon (also at the bosses). Fps drops to ~5-15 and it gets unplayable.

See if it's for some reason set itself to XP compatibility mode...
AMD Radeon HD 6650M and patch didnt solve my problems (its esp. at bosses that spawn tons of adds)
I've also watched on my Windows Task Manager and my Page File Usage History is at ~73% all the time, Maybe that has something to do with it? :<

Check your particle display options. Turning them down might help if you are with classes or fighting mobs whose attacks tend to be flashy. Are your video settings at the recommended level? Make sure you clear out your Game Install Folder > Neverwinter > Live > Logs > Gameclient folder from time to time before a new play session. Those files are appended to and not overwritten.

recommended graphics are highest but that would only make it worse? (got render scale and gaphics on lowest quality atm)

Yeah, if they are higher, it would only make it worse. Was just making sure your graphics set higher then the recommended at least.
