I've encountered this problem with frustrating regularity. If a player DCs, freezes, or something happens that causes you to idle for 15 minutes, or even if you decide to log off, progress in your current instance is lost. I've never encountered this before, but this can be very morale breaking, especially if I'm an hour into a long dungeon.
The first time it happened, I was on the mission where you have to beat the spell plague elixir guy. You start in town, go through the sewers and finally up his tower. About half way through the sewers my roommates asked if I wanted to do an inhouse in League of Legends. I said that I would as soon as I found a campfire, thinking that since it's a respawn point...that would be where I respawn at when I log back in. But I had to start over.
The most recent time - I couldn't sleep so I thought I'd play through a quest or two. I was perhaps a half an hour into clearing out drow infested tunnels when my power flickered. Knowing what would happen, I just went to bed. When I logged in this morning, my rogue was laying dead outside of the instance.
I really think this should be addressed somehow, simply because there is no option to just leave your character on; the game will boot you. Also there is really no telling how long an instance will be before a person starts. Some are little more than 5 minutes and others can take over an hour. Real life happens and I think it should always be considered.
