Lair of the Mad Mage Not unlocking after completing Undermountain for some players

Undermountain fully completed but the big, red lock is still present right next to the Lair of Mad Mage dungeon. I thought it was an isolated problem but so far 5 other people have reported having the same issue on my guild alliance alone.
