Game crash when entering dungeon ?

the game crashes quite a lot when i try to enter a dungeon or occasionally just going from map to map. it used to be about 1 in 5 but is now 2 in 5 times, i then have to log back into the game and it asks me if i want to login normally or safe login. Anybody else having this issue or anyone got any ideas ?

Yes happens to me which can be annoying trying to get back quickly if in a party.

This has been happening for months. There's ALWAYS someone who DC's going into a map in group content.
