Unable to slot Companion Gear

On the top slot, I seem to be having trouble slotting companion gear even though I have one. I can only slot them on the middle and bottom.

The companion gear is a +5.

    Is it a piece of gear the companion can use there?

    Off course, it is, it's a Heroic Talisman of the Companion +5....

    Check the image and compare it with the one that is slotted.
