As soon as someone declines (you see the red icon on one of the 5 entries), you pretty much know with certainty that the instance will fail. Once someone declines the "queue system" is perfectly happy to take anyone around to fill the slot, even if the person who declined was the healer or tank.
And DEFINITELY do not load me into an epic dungeon with a two-man party. If it's not a 5-man party loading in, cancel it and re-queue me as the odds of getting a party that can succeed when you start with 4 or fewer (especially 4) is abysmally small.
Making it an auto-fail and requeue won't really be any more griefable than it already is. Parties missing people or missing essential roles are just a waste of time. Both in actually trying to run it, and in sitting there waiting for it to load you up into a pointless instance. Making anything that makes that red icon on the queue pop-up result in an auto-fail will just save us all this wasted time and frustration.
Either that or fix it so that only healers can replace healers, only tanks can replace tanks, and only full groups can load up a new instance (fail and requeue if not and the timer expires), and do some basic checks to know if an OP is even remotely capable of tanking or healing (the game seems to count them as both simultaneously and indiscriminately, which can leave you in a dungeon that needs a tank or healer but has an OP that's the squishiest guy in the party and can't heal for beans--in part because he spends most of his time dead).
