Though I'm sure there have been threads about this for, I can't seem to find any.
Occasionally during fleet ops like Halting the Gorn Advance or secure system missions, my ship will be destroyed but instead of respawning the camera will stay along side my destroyed ship. I can leave the instance and fix it, but that usually means I have to lose out on finishing that mission or getting any type of reward. As soon as I leave the system my ship is still destroyed and need to respawn again.
