Yes. The upgrade token can be applied to any T5U and T6 ship.

It's true for all ships, you add +1 console even if it had 11 or 12.
One thing to keep in mind is if a ship also has a fleet version, you want to buy the fleet version first and apply the token to that not to the regular version.
The regular and fleet ships are completely different ships in the view of the game. Upgrading the regular does not upgrade the fleet and vice versa.
I believe this is the reason the Lexington is so popular. It's a Miracle Worker with the extra slot and it is already Fleet quality, and has a pet. One of the best ships in the game and it's a Zen ship too boot! On my free account, I used the free t6 token from the anniversary giveaways to get it and upgrade it to t6-x, totally worth it for absolutely free!

Yep, but just to confuse people you still need to upgrade each character separately but the cost changes to 0 instead of a token after the first one.
