[PS4] EPISODE COMPLETION / TEAMING ISSUES

As a veteran PC player, I'm leveling a Console toon with a friend, being a wise Jedi to his STO Padawan. For the THIRD time now, I've failed to complete an episode - after we progress through and complete all objectives! (Naturally, it's the most annoying episodes: The Vault, Operation Gamma, Facility 4028).
My question is this:
How many things do I need to do (aka interact with) in order to have the game register me as completing the episode? I've mostly been taking the backseat and letting my teammate "drive" so he can read all the story and instructions, since I've already leveled 6 toons on the PC side.
Objectives all "go green" on the side journal and we progress through an episode, but when complete, I do not get the "Hail to Report to Starfleet/Completion" -- only the same episode to start with the first objective in the episode listed as white.
Trying to figure out exactly what trigger this "play-thru of episode but don't get credit for yourself" glitch... is it because I'm not Team leader? Is it because my teammate shared his mission with me? Is it because I have the same episode listed as active as he does? Is it because the same episode in my journal may or may not be listed as Primary (even though it's an active episode)??
