Yah, it went like this, i lunged at him, and he was blown back off the catwalk, he raised over the bar and I raised up to kick, then he fell back over and i'm doing the kick all the way down... it was like Lui Kang's kick in Mortal Kombat...
I was doing a Breach run with my fleet mates. I beamed in from ESD and forgot to change my ship from my PvP healing Ambassador to my PvE Chel Gret build. Figured it'd be fine, at least I wouldn't die......
So we get into the first room in the ship and I hit my Rommie-clicky-cloak to blitz to the back left and save the two ships there and close the doors. No problem, I saved the ships and closed the doors then went to swing my ship around and hit the Bastions that are in that corner....forgot it wasn't a Chel Gret and got hung up in the wall in the corner. I was trying to get my ship out of there, but floundering and getting hammered by the three Bastions and 6 Frigates that are there. I popped all my heals and switched to engine mode (I burned the evasive maneuvers to get past the first ship and save the second). Still stuck on the wall and one shield facing down, my fleet mate in a bug swooped in and took out 3 of the 4 ships in front of me with an Alpha, Scatter Volley and a torp spread....3 Warp core breaches later, I am a grease spot.....D'OH!!
Step Between Stars, fighting the Voth. I selected the beam things for support.
First wave went good. Tuvok was knocked unconscious on the second wave. Third wave, I hit security escort, my horta, and shard of possibilities. I'm pretty sure all my doffs procced, because a glorious army appeared and marched into battle ahead of me...
Right into the beam, vaporizing them all and leaving me to die alone, unable to fight back in shocked disbelief.
In "A Step Between the Stars", I went to interact wit the second vent console. Didn't notice the energy buildup and the big energy beam fired. It barely missed me, but I did the only sensible thing: I backed up straight into the beam for instant vaporization. My Admiral apparently has the observational skills of a willing redshirt.

When I look back, my Vor'cha is a fireball and the number "127 000" is floating above the wreck. :P
Did my first HGE with my fleet. "Don't stand on the glowy green squares." Check. First thing I do, not pay attention and run onto said square. Instant dead. I am a genious that for some reason was given command of a ship by Starfleet

In PvP, I was cloaking after an Alpha strike (shields up, was using distributor unit) then a BO3 elachi DBB procced killing me from nearly full health. Bad words were said that dreaded day.
I've came back in wrath and facecritted that guy, had to kill him back.
Battlecloaking in front of a Borg gate, with a plasma torp coming my way. Facepalm.
This is why I will never take the console that allows me to stay shielded while cloaked off my Scimitar. Min-maxing be damned.
As for my own lame deaths, I've died twice since the launch of season 9 to the Undine ground AOE, as a result of tunnel vision whilst aiming.

my death and the npc nicor ship useing the planet killer attack which was funny as heck i was fighting another undine ship when one comes behind me and hits the planet killer console right onto my ship apparently the step between stars shields either glitched or it works with that attack repelled the attack right back as phaser damage onto the ship took the ship out and somehow the warp core breech then took me out <_< >_>

This one happened several years ago. I entered a pvp match without knowing (I don't pvp and at the time I had no idea that you would enter pvp matches like that) with my new Defiant. Although I don't want to pvp I though "ok, since I'm already here". I was paired with a Galaxy. The other team was a Jem Dread plus two JAS. Totaly inexperienced as I was I thought that if I cloaked and fired at point blank I could take the dread. So I did. Decloak, alpha strike...his shields didn't even drop. Next second, they all fire at me and BOOM!. Lame...
Yesterday, after blowing up the planet killer. Cutscene (or was it the marks box selection? i think it was a cutscene). I go Boom from the explosion, because the damn game didn't let me evade. Lame...
I remember being Captain, and my team member, a VA, were doing a PvP against these two other people (one escort, one cruiser).
The VA finished off the cruiser (which exploded and damaged the second one) ... only moments later being destroyed by the second one. I used ramming speed (he used an AoE attack ... had me on ~47%) and hit him, only to survive on 3%. He then started blowing up, and I hastily tried to run away. I turned the camera around to look at the glory ... but ... I then crashed into an asteroid ... only to wait for the inevitable explosion to kill me.

I am a fleet leader (of two fleets). So the other day I'm recruiting and have a bunch of new to the game, new level 50 players ready for their first conduit run. I am in my scimitar and they in mostly mirror ships, one in a vesta as I recal.
My scimi is nothing OP as some but a great pve build, my teammates buing relatively new were impressed and compliments and questions about my build made my head swell a little.
So we marched on, the other guys (bar the guy in the vesta) all die a few times but we do not, killed the tac cube at the end (you know where this is going right?).
So a guy asks me what weapons I have, and being too busy dropping TRIBBLE into fleet chat and typing how to get them I forget aaaaalllll about the tac cube going critical right in front of me.
I go boom and the guy in the vesta on his first run, with standard gear lives as he is actually paying attention and out of range.
Not my dumbest death (ill think of more if this thread keeps up) but certainly embarassing lol!

Doing a borg ground elite (a while ago now so I forget which one).
I am not a big ground player and got a senior member of our fleet and a few others to give me a learning run.
It went ok and we won best as I recall but I did get assimilated and gunned down by my own men. Blasted off a ledge with kB lol! I think I remember "it was for the admirals own good" in fleet chat by @extremekillz who was the one to send me on my wonderful flight
