I have done this mission before on my FED and Romulan character with no issues.
I am now attempting to do this on my Klingon engineering character. I have tried over 6 times to defeat the 3rd opponent in the arena. Some big white ape. My shields are ok, but my health gets killed off quick by this beast.
What am I doing wrong?
I had no issues on my other toons.
Use the healing roots near the outside of the arena :P It's much harder now I agree, you've just got to chip at him. Try doing a combo attack, then retreating a short distance, then do it again (having lunge in your kit also helps)

I have tried everything. "Hit and run". Apparently shields are ignored in this, only health is affected. I have ran around that arena looking for anything to restore my health with no luck.
It is just funny how I breezed through with my FED and Romulan without any issues, but I go in with my KDF I get killed always.

Here is one thing I noticed. For the first time prisoner 1478 entered the arena with me.
All the other times where I kept getting killed, he would stay near the cells.
What kills you? That Mukato is new. Let Slamek take aggro and flank the opponents. This is melee combat, you probably don't know there are combos.
1 1 2 gives a 300% crit chance. 1 2 2 is a spin attack that does AeO damage, perfect against multiple enemies.
The big worm can hit you hard. I too got killed in the arena. As soon as he spawns you can activate the turrets. The panels are on both sides of the arena. Try to be there as he spawns. The smaller worms spit too, but you can evade that. Don't go straight at them, go left then right or right then left.
Now if you use a one hander, a sword, the preferable weapon though, you have a kind of a lunge. As soon as your attack button lights up, (1) you can activate it and you will sprint to your target.
When the big worm spawns for the final fight, free the other prisoners. They are behind the doors where your cell block is. If necessary go into the gangway and take some cover. You can heal up there. Sprint to the other side of the arena and activate the turrets. That should do it.
