The helmet is on the mk XII MACO, Omega Force, and Honor Guard set costumes.
You earn the sets from special task force missions. You can get mk X or XI versions from currencies you collect on normal difficulty, mk XI has a costume unlock without the helmet. In Elite ground STFs you can get the mk XII version, which has the full helmet unlock. To unlock the costume, you have to get all three pieces (personal shield, armor, weapon) at the right mk level.
The MACO set is Federation only, the Honor Guard set is KDF only, and the Omega Force set is available to both factions.
There's a couple recent stickies about STF loot on this forum with some good specifics on how to get the sets.

I just got the last of the 3 piece OMEGA MK XII set but helmet is not showing up did they change when you unlock the helmet in the past couple of patches. The only thing new that unlocked was shoulders and collar.

The helmets appear under head options and not the costume options. Check Head Type for the helmet.
I wish you could get helmets or helms in lower stages. It looks weird that you have all this armor & nothing but an invisible shield to protect your head.

mostly for visibility reasons i'd guess, since until the Eva suits came around, there Really was no need for a fully sealed suit and helmet. the omega/maco is just for looks, only the EVA suits serve as a functional helmet.

Remember the part with the helmet at the beginning of Saving Private Ryan?
I would love more EVA suit options or costumes from the start of the game. Maybe add the suits from First Contact to the costume pack already in the C Store? And add the TOS suits to that bundle...
