I just started playing STO yesterday and I've been enjoying particularly space missions so far. Unfortunately it seems like I usually can't take more than ten steps in ground missions (plus Earth Dock) between brief but really annoying stutters, that looks like a few skipped frames of animation or a mini version of latency related rubber banding.
Since Fraps or /showfps don't show any drops in framerate when those stutters happen, I'm leaning towards this actually being rubber banding, but I've had no apparent lag in space missions - or delays when clicking interface buttons in ground missions - and with /netgraph turned on the green graphs looks pretty even and the ping fluctuates around 200 ms (not exactly great, but unless there's some packet loss going on as well it shouldn't really explain lag stutters. First thing try going to your Nvidia Control panel, then to manage 3d settings, in manage 3d settings change Max pre-rendered frames from 3 to 0. Also can you post a dxdiag?
To create a DXDiag report, follow these steps:
1. Click "Start" menu, then click "Run"
2. In the "Open" box, type: DXDiag (hit enter)
3. If this is your first time running DXDiag, click "Yes" to launch the DirectX Diagnostic Tool.
4. Click "Save All Information" to create a DXDiag report.
5. Choose a name and location for the dump file (recommend desktop) and click "Save".
I know this is really going to sound like a silly question but did you install the new drivers over the top of the old ones? If so, it may explain why you are still seeing some of the stuttering so here are the steps I usually use in order to remove older NVIDIA drivers:
a. Go into the Control Panel and under Add/Remove Programs (or Programs and Features in Vista and Win 7) find the driver in the application listing.
b. Left-click on the driver labeled NVIDIA Driver. Once highlighted click on the Uninstall button (or Change/Remove under Windows XP) up at the top which should now start the full uninstall process. When the uninstall completes do NOT reboot your system yet.
c. Look for two other products such as the PhysX driver and Nforce Display Manager and make sure they are both uninstalled as well.
d. Once you have the NVIDIA Drivers, NVIDIA PhysX, and NVIDIA Nview manager removed go ahead and reboot your system.
e. After your system finishes rebooting, and you cancel the "Install driver" messages, you will run the driver that you saved to your desktop; making sure to reboot your system once the new driver has finished installation.
If you already took those steps and are still suffering from the issue chances are someone is going to ask you for your traceroute results using the command tracert patchserver.crypticstudios.com. Now do be sure to copy the results into your post once you have them.
Also, go to http://files.champions-online.com/nettest.exe, download this file and run it. A command prompt should appear running the test automatically and make sure to post those results here as well.
I always uninstall all the Nvidia items before upgrading drivers, but after the first required restart, Windows always instantly restores the original Nvidia driver that came pre-installed on the PC (version 182.x if I recall correctly).
After that I proceed to install the new driver, and afterwards the Nvidia control panel shows the new driver version as you would expect. If there have been any leftovers from a previous driver version after an update, it's certainly not something that has ever caused my any issues in other games - online or singleplayer.

I tried changing the Max pre-rendered frames from 3 to 0 as suggested, but it made absolutely no difference in the few minutes I just tried in a ground mission. Heavy stuttering - without framerate drops (it stays in the 30-40+ range) - while my character is running, but no indications of noticable network related lag during combat or interface use.
There's a very big visual difference between low framerates (even if they were slideshow low, which even 30 is obviously not - that's what you get in most console games) and the type of stuttering I'm talking about here.
This looks a lot more like "micro rubber banding" during movement, but - as mentioned - without any other obvious indications of network lag.
As for the framerate in general, I am a bit dissatisfied with the sometimes only around 30 fps I get on the ground, and that's with a few settings turned down a couple of notches (many settings don't really seem to make any difference in performance at all though).
In space missions (and since I'm just starting I haven't been in any huge fleet battles yet) I generally get 40-60+ fps, and 60+ on the sector map.
I haven't had vsync turned on, but I had actually been meaning to give that a try today, not that I expect it to make any difference in relation to the stuttering.
I just played a ground mission and I'm starting to think this really is network related rubberbanding. The stuttering fluctuated in severity and frequency throughout the mission, getting particularly bad right before a brief "server not responding" flashed at the top of the screen at one point.
At other times I could go for almost a full minute without any movement stuttering at all.
I also managed to get the framerate up in the 50+ range most of the time by simply turning off screen space ambient occlusion. For an extra ~15 fps that's a graphical trade off I can certainly live with, as opposed to turning off some of the lighting settings which also have a pretty big impact on performance, but make the graphics look rather horrible without them.
Turning vsync on didn't seem to do anything but what it's intended to do, eliminate screen tearing (and cap my fps at 60).
It really sucks if I do have to suffer with network lag or packet loss in those ground missions - and seemingly only there as space continues to run close to flawlessly.
I'm on a usually very stable 50 Mbit cable connection - and play a racing game regularly on US servers with an only ~133 ms ping and zero lag. Just about everything else I do online suffers from no or minimal lag as well (although in the case of online games I guess most of them run off EU servers for European players).

Can you post the results to this test, as your dixdiag didn't show any real issues.
Run a trace route:
* Click on Start
* Click on Run, or if using Windows Vista just skip to the next step
* Type cmd into the dialog box and press Enter.
* In the window that opens type "tracert patchserver.crypticstudios.com"
* Check the speed listed on the return
If you see any hops over 180ms, this could be the cause of the rubber banding.
How to acquire, run and read the Nettest tool:
Go to http://files.champions-online.com/nettest.exe
Download the application and run it (Windows 7 and Vista users make sure to right click the icon and run as admin if you are having issues with the tool running.)
The Nettest tool checks that the ports required for patching and playing Star Trek Online are not blocked or filtered. Good values returned from the Nettest tool look like:
Column 1 and 3 have values around 500 kb/sec (outgoing/incoming), column 2 has values around 20 kb/sec (transfer rate). If the values in these columns is less than what is expected you may have filtering or blocked ports either on your personal network or from your ISP.
I do not see anything in your nettest that would indicate a networking issue.
Have you tried changing your Maximum pre-rendered frames? To do this open the Nvidia control panel, got to manage 3d settings, highlight Maximum pre-rendered frames, and set it to 0. This may change things but it may not, but its worth a try.
Have you tried running stress test applications to make sure you card is healthy? You are running a 5 year old card, and there could be the chance its on its last legs.
Actually, I picked up a nvidia gtx 470 card today. AND it still has that little stutter, freeze going on. Weirdest darned thing.
I have noticed that the 197 drivers have less stutter than the new 258.69 drivers.
As far as tweaking settings, I am notorious for tweaking them until I get what I want. I have tried both lowering and upping the pre-rendered frames. Moving settings up and settings down.
IF I want an ugly looking game on the low settings I would still have this stutter-freeze (just tested it on the new card)
What is strange, Is I have played since closed beta and all the fun we had with Nvidia drivers. With the new drivers from nvidia and the improvements to the game it just kept getting better and better, and suddenly "POOF" it goes to TRIBBLE.
CO and STO are the only games that do this. SWG, WOW, LOtR, GW, CoH all run just fine, and even better with the new card. BUT not co and STo.

with your system you should easily make 60 FPS with VSync active, do you have set any fps limits?
( not counting fleetactions, they bring every systems FPS down ;P )
This looks a lot more like "micro rubber banding" during movement, but - as mentioned - without any other obvious indications of network lag.
As for the framerate in general, I am a bit dissatisfied with the sometimes only around 30 fps I get on the ground, and that's with a few settings turned down a couple of notches (many settings don't really seem to make any difference in performance at all though).
In space missions (and since I'm just starting I haven't been in any huge fleet battles yet) I generally get 40-60+ fps, and 60+ on the sector map.
I haven't had vsync turned on, but I had actually been meaning to give that a try today, not that I expect it to make any difference in relation to the stuttering.
At other times I could go for almost a full minute without any movement stuttering at all.
I also managed to get the framerate up in the 50+ range most of the time by simply turning off screen space ambient occlusion. For an extra ~15 fps that's a graphical trade off I can certainly live with, as opposed to turning off some of the lighting settings which also have a pretty big impact on performance, but make the graphics look rather horrible without them.
Turning vsync on didn't seem to do anything but what it's intended to do, eliminate screen tearing (and cap my fps at 60).
It really sucks if I do have to suffer with network lag or packet loss in those ground missions - and seemingly only there as space continues to run close to flawlessly.
I'm on a usually very stable 50 Mbit cable connection - and play a racing game regularly on US servers with an only ~133 ms ping and zero lag. Just about everything else I do online suffers from no or minimal lag as well (although in the case of online games I guess most of them run off EU servers for European players).
I am getting great frame rates in space and ground, mediocre at Earth starbase. However, with no change in internet latency, ping, and no change in frame rate reported by the game and a third party frame rate widget, my toon (space and ground) will just freeze in place for up to 2 seconds. And occasionally will rubberband after a freeze.
I am not sure what is causing it, other than it is something to do with the the game/graphics driver.
It almost feels like the game/driver is having to do extra work to load something, like a texture, a particle, or a shadow.
It is damned frustrating. And has lead to a big decrease in game play for me.

How to acquire, run and read the Nettest tool:
Go to http://files.champions-online.com/nettest.exe
Download the application and run it (Windows 7 and Vista users make sure to right click the icon and run as admin if you are having issues with the tool running.)
The Nettest tool checks that the ports required for patching and playing Star Trek Online are not blocked or filtered. Good values returned from the Nettest tool look like:
Column 1 and 3 have values around 500 kb/sec (outgoing/incoming), column 2 has values around 20 kb/sec (transfer rate). If the values in these columns is less than what is expected you may have filtering or blocked ports either on your personal network or from your ISP.

I do not see anything in your nettest that would indicate a networking issue.
Have you tried changing your Maximum pre-rendered frames? To do this open the Nvidia control panel, got to manage 3d settings, highlight Maximum pre-rendered frames, and set it to 0. This may change things but it may not, but its worth a try.
Have you tried running stress test applications to make sure you card is healthy? You are running a 5 year old card, and there could be the chance its on its last legs.
I have noticed that the 197 drivers have less stutter than the new 258.69 drivers.
As far as tweaking settings, I am notorious for tweaking them until I get what I want. I have tried both lowering and upping the pre-rendered frames. Moving settings up and settings down.
IF I want an ugly looking game on the low settings I would still have this stutter-freeze (just tested it on the new card)
What is strange, Is I have played since closed beta and all the fun we had with Nvidia drivers. With the new drivers from nvidia and the improvements to the game it just kept getting better and better, and suddenly "POOF" it goes to TRIBBLE.
CO and STO are the only games that do this. SWG, WOW, LOtR, GW, CoH all run just fine, and even better with the new card. BUT not co and STo.
Yeah unfortunately I Do not you can run these commands to kind of narrow things down. Just look for massive fluctuation and changes.
